女性生殖道沙眼衣原体感染对IVF-ET新鲜周期和复苏周期妊娠结局的影响

【摘要】 目的探讨女性生殖道沙眼衣原体(chlamydia trachomatis,CT)感染对体外受精-胚胎移植(in vitro fertilization-embryo transfer,IVF-ET)新鲜周期和复苏周期妊娠结局的影响。方法选取2016年8月至2017年10月在武汉同济医院接受IVF-ET治疗之前行CT检测的女性患者,共计3654个新鲜周期和2766个复苏周期,根据CT感染、治疗情况和复查结果将其分为转阴组、未转阴组、未治疗组和阴性对照组,比较患者的一般临床特征和妊娠结局。结果 IVFET新鲜周期中,转阴组和阴性对照组的受精率、囊胚形成率差异有统计学意义(均P<0.05);未转阴组和阴性对照组、未治疗组和阴性对照组的卵母细胞MⅡ率差异有统计学意义(均P<0.05);妊娠结局在各组之间的差异无统计学意义(P>0.05)。复苏周期中,各组患者的妊娠结局差异无统计学意义(P>0.05)。结论女性生殖道CT感染对IVF-ET新鲜周期和复苏周期均未产生不利影响。

【Abstract】 Objective To investigate the effect of chlamydia trachomatis(CT)infections of the female genital tract on pregnancy outcomes of fresh and frozen-thawedin vitrofertilization-embryo transfer(IVF-ET)cycles.Methods Female patients who underwent CT examination before the IVF-ET treatment in Tongji Hospital from August 2016 to October 2017 were enrolled in the study(a total of 3654 fresh cycles and 2766 frozen-thawed cycles were included).According to the CT infection,treatment and retesting results,they were divided into negative-conversion group and persistent positive group,untreated group,and negative control group.The basic characteristics and pregnancy outcomes were compared among these groups.Results In fresh IVF-ET cycles,the fertilization rate and blastocyst formation rate were significantly different between negative-conversion group and negative control group(P<0.05);also,there was significant difference in the MII oocyte rate between persistent positive group and negative control group and between untreated group and negative control group(P<0.05);no significant difference in pregnancy outcomes was found among these groups(P>0.05).In frozen-thawed cycles,there was no significant difference in pregnancy outcomes among these groups(P>0.05).Conclusion CT infections of the female genital tract have no adverse effects on the pregnancy outcomes of either fresh or frozen-thawed IVF-ET cycles.
