经皮血气监测在危重症中的临床应用

【摘要】 血液通过皮肤与外界环境空气进行气体交换称为"皮肤呼吸",这是经皮氧分压监测的理论基础。经皮无创血气监测技术是通过CLARK电极对皮肤加热,使探头所在部位的皮下毛细血管血供增加,动脉血和表皮下毛细血管产生气体交换,毛细血管动脉化,氧从毛细血管弥散到皮下组织、皮肤,进而监测到PtcO2的技术,其具有无创、实时监测、反映组织氧及二氧化碳分布等优点。大量文献证明经皮氧分压(PtcO2)、经皮二氧化碳分压(PtcCO2)与PaO2、PaCO2有很好的相关性,能够反映患者的PaO2/PaCO2的即时变化。因此,PtcO2、PtcCO2的连续监测是临床诊疗重要的参考指标,具有广阔的临床应用价值。

【Abstract】 The exchange of gases between blood and ambient air via the skin is called " skin respiration",which is the theoretical of monitoring the percutaneous oxygen partial pressure( PtcO2). Percutaneous non-invasive blood gas monitoring technology heats the skin by the CLARK electrode and then the blood supply of the subcutaneous capillary in the probe is increased,this resulted that the gas exchange between the arterial blood and the subepidermal capillaries and the capillary arterialization,the diffusion of oxygen from the capillaries to the subcutaneous tissue,skin,and then the technology of PtcO2. It has the advantage of non-invasive,real-time monitoring,reflecting the distribution of tissue oxygen and carbon dioxide. A large number of studies have shown that PtcO2 and transcutaneous carbon dioxide partial pressure( PtcCO2) have a good correlation with PaO2 and PaCO2,which can reflect the immediate changes of PaO2/PaCO2 in patients. Therefore,continuous monitoring of PtcO2 and PtcCO2 is an important reference indicator for clinical diagnosis and treatment,and has broad clinical application value.
