经桡动脉冠状动脉介入诊疗术后桡动脉损伤的影响因素评价
Evaluation of Influencing Factors of Radial Artery Injury after Radial Artery Interventional Treatment
【摘要】 目的验证经桡动脉冠状动脉介入诊疗术后桡动脉损伤的影响因素。方法 56例研究对象均选自广西省百色市人民医院2016年10月—2017年10月接收的经桡动脉冠状动脉介入诊疗术患者,将其随机分为观察组和对照组,各28例。观察组为5F动脉鞘管组,对照组为6F动脉鞘管组,观察两组患者术后桡动脉损伤的影响因素。结果观察组的患肢疼痛、前臂血肿、桡动脉内膜增厚、RAO、RAS等状况明显低于对照组,差异有统计学有意义(P<0.05);两组患者都未出现假性动脉瘤、前臂大血肿、骨筋膜室综合征和动静脉瘘等状况;另外,观察组术后压迫止血时间为(4.25±0.45)h,明显少于对照组的(6.35±0.85)(P<0.05)。结论采用直径小的动脉鞘管进行桡动脉冠状动脉介入诊疗术,可有效减小RAO的发生,防止术后压迫出血状况,并降低桡动脉内膜损伤程度,使患者的舒适度提升。
【Abstract】 Objective To verify the influence factors of radial artery injury after percutaneous coronary intervention.Methods 56 cases of this study were selected from Baise People’s Hospital from October 2016 to October 2017.They received radial artery interventional diagnosis and were randomly divided into observation group and control group, 28 cases in each group.The observation group was the 4F artery sheath group and the control group was 6F artery sheath group. The influence factors of the two groups of patients with radial artery injury after operation were observed. Results Limb pain, forearm hematoma,radial artery intimal thickening, RAO and RAS status in observation group was significantly lower than those in control group,with statistically significant difference(P<0.05); Pseudoaneurysm, forearm hematoma, osteofascial compartment syndrome and arteriovenous fistula in two groups did not appear. In addition, postoperative hemostasis time in observation group was(4.25±0.45) h, which was significantly less than(6.35±0.85) h in control group(P<0.05). Conclusion The use of small diameter artery sheath catheter for radial artery intervention can effectively reduce the incidence of RAO, prevent postoperative compression hemostasis, reduce the degree of intima injury of radial artery and improve the comfort of patients.
