关节镜下自体腘绳肌腱与同种异体肌腱重建前交叉韧带疗效及免疫排斥情况观察
Clinical outcomes of arthroscopic anterior cruciate ligament reconstruction considering immune rejection:autograft versus allograft
【摘要】 目的观察关节镜下自体腘绳肌腱与同种异体肌腱重建前交叉韧带临床疗效及术后免疫排斥情况。方法前瞻性将2012年1月至2016年12月间收治的膝关节前交叉韧带损伤的患者84例按照随机数字表法随机分为A组(自体腘绳肌腱组)及B组(同种异体肌腱组),A组44例,B组40组。A组植入患侧膝的腘绳肌腱,B组植入同种异体肌腱(胫骨前肌),行前交叉韧带重建。术后,对两组患者膝关节功能(Lysholm评分、Tegner评分、IKDC评分)、Kneelax关节动计度测量仪测量膝关节稳定性、Lachman试验及免疫排斥反应发生情况进行评价。结果 (1)与术前比较,术后两组Lysholm评分、Tegner评分、IKDC评分升高(P<0.05),Kneelax评分降低(P<0.05),但两组间比较差异无统计学意义(P>0.05)。Lachman试验:术后Lachman试验两组间差异无统计学意义(P>0.05)。(2)B组免疫排斥反应发生率比A组高,但差异无统计学意义(P>0.05)。结论和腘绳肌肌腱重建前交叉韧带相比较,同种异体肌腱有着相似的临床疗效,两者免疫排斥反应均不大,临床上患者可根据自己的要求选择。
【Abstract】 Objective To observe the clinical outcomes and immune rejection of anterior cruciate ligament reconstruction using tendon autograft versus allograft.Methods 84 inpatients with anterior cruciate ligament injury in our hospital from January 2012 to December 2016 were randomized into two groups,i.e.,group A(hamstring tendon autograft) and group B(allograft tendon).After surgery,knee function(Lysholm,Tegner,IKDC scores),knee stability(Kneelax measurement),as well as the immune rejection were assessed and Lachman test was undertaken.Results(1)After surgery,Lysholm,Tegner,IKDC scores of the two groups were higher(P < 0.05) while Kneelax score was lower postoperatively(P < 0.05),but there were no significant differences between the two groups(P > 0.05).Lachman test also showed no significant differences between the two groups after surgery(P > 0.05).(2)The rate of immune rejection in group B was higher than group A,but without significant differences(P > 0.05).Conclusion The efficacies of autologous and allogeneic arthroscopic cruciate ligament reconstruction were similar and with almost equivalent immune rejection,leaving surgical option more to the patients.
