3种方法去除根管玷污层能力的比较研究

【摘要】 目的:比较超声、声波和Er∶YAG激光去除根管玷污层能力。方法:将离体单根管的下前磨牙40个,分别用声波器械(YB组)、超声波器械(CS组)、Er∶YAG激光(Er组)以及生理盐水(DZ组,对照组)行根管预备后的辅助预备(n=10)。记录根尖孔碎屑溢出量。应用扫描电镜进行观察根管内侧壁,根据根管表面玷污层情况和牙本质小管腐蚀程度比较3种方法去除根管玷污层的能力。结果:4组根尖孔碎屑溢出量无显著性差异(P>0.05)。在去除玷污层方面,CS、YB和Er组优于对照组(P<0.05);在根上、中1/3,CS、YB和Er组间比较,P>0.05;在根尖1/3,CS、Er组效果好于YB组,但差于本组根上、中1/3(P<0.05)。在根管壁腐蚀程度方面,CS组在根上、中1/3出现腐蚀,多于其他组(P<0.05)。结论:Er∶YAG激光去除玷污层效果较音波好,与超声近似,尤其根尖1/3区,对根管壁表面结构无明显破坏,适宜临床常规应用。

【Abstract】 Objective: To compare the capability of root canal smear layer elimination in root canal preparation among ultrasonic,sonic and Er: YAG laser. Methods: 40 extracted mandibular premolars were randomly divided into 4 groups( n = 10),and treated with ultrasonic( group CS),sonic endo instrument( group YB),Er ∶ YAG laser( group Er) and normal saline( control group) respectively as the auxiliary method for canal preparation. The amount of apical extruded debris were recorded by the scan electron microscope. The amounts of smear layer and dentinal morphostructure in root canal,the cleanliness of root canal and the erosion of the canal dentin were measured and analyzed by one-Way ANOVA. Results: The amounts of apical extruded debris were no significant different among the groups( P > 0. 05). In the coronal and middle thirds,no significant differences were observed among CS,YB and Er groups. In the apical third,CS and Er showed significantly better smear layer removing ability than YB. In the coronal and middle thirds,ultrasonic group showed more significant erosion than the others. In the apical third,there was no statistical difference among the 4 groups. Conclusion: Er ∶ YAG laser is more efficient than sonic endo instruments in the removal of smear layer from the root canal system and similar to ultrasonic especially in apical third. Er ∶ YAG laser does not cause lesion on canals dentin.
