双能CT Rho/Z定量分析体内上尿路钙结石亚型的初步研究
Rho/Z quantitative analysis sub-differentiation of upper urinary calcium calculi on dual-energy CT:a pilot study in vivo
【摘要】 目的 探讨双源双能CT电子密度(Rho)及原子序数(Z)定量分析体内上尿路钙结石亚型的价值。方法 共纳入55例钙结石,结石离体前均行双源CT双能量模式扫描,以红外光谱分析结果作为金标准,分为4组:组1[一水草酸钙(COM),12枚],组2[COM+二水草酸钙(COD),9枚],组3[COM+碳酸磷灰石(CaP),8枚],组4[COM+COD+CaP,26枚],最后将组2、组3、组4融合为新的1组,为混合钙结石组。通过获得的Rho/Z,对不同类型的钙结石进行定量分析。结果(1)4组比较时Z有统计学差异,而Rho则无统计学差异;两两比较时,组4与其他3组的Z有统计学差异。(2)COM的Z低于混合钙结石,受试者工作特征曲线(ROC曲线)分析COM的曲线下面积(AUC)为0.738,截取值为12.38。结论 双源双能CT成像技术获得的Z有助于区别不同类型的钙结石,在术前评估中具有重要价值。
【Abstract】 Objective To explore the value of the electron density(Rho) and atomic number(Z) quantitative analysis sub-differentiation of upper urinary calcium calculi on dual-energy CT in vivo.Methods Totally 55 calcium calculi underwent dual-source CT in dual-energy mode were analyzed retrospectively.According to the result of infrared spectrometer,four types of calcium calculi were seen in this study:group 1(COM,12),group 2(COM+COD,9),group 3(COM+CaP,8),group 4(COM+COD+CaP,26),then combining group 2,group 3and group 4 was named mixed calcium calculi group.We used the Rho/Z quantitative analysis sub-differentiation of calcium calculi.Results(1)The parameters of Z measured by four groups were significantly different,however,no significant differences in the parameters of Rho were found.The Z of group 4 with other three groups was different significantly.(2) The COM group had significantly lower Z compared with mixed calcium calculi group,and ROC analysis for detection of COM calculi yidlded AUC of 0.738 with cut-off 12.38.Conclusion The parameter of Z is helpful in the characterization of various types of calcium calculi,and has important significance in the preoperative assessment.
