IgG4相关性肺病的MSCT影像表现分析

【摘要】 目的 探讨IgG4相关性肺病的多层螺旋CT(MSCT)表现,以提高对其认识和诊断水平。方法 回顾性分析5例经手术或穿刺活检病理证实的IgG4相关性肺病的临床及MSCT资料。结果 5例IgG4相关性肺病均表现为肺内单发结节或肿块,大小为17.6 mm×13.6 mm×49.5 mm~51.8 mm×38.0 mm×37.7 mm;病灶-肺界面均清楚、边缘呈浅分叶状,可见毛刺;包绕支气管、血管生长,平扫均为软组织密度影夹杂低密度影,增强扫描低密度影无强化,软组织密度不同程度强化;宽基底与胸膜相连,周围可见纤维条索病灶;同侧肺门及纵隔可见多发淋巴结肿大。结论 IgG4相关性肺病表现为肺内单发结节、肿块,边缘浅分叶,可见毛刺,周围见条索影,病灶内见阳性支气管征、血管包埋征等,软组织密度均匀,增强不同程度强化等特征。

【Abstract】 Objective To analyze the MSCT imaging features on IgG4-related lung disease,and to improve the understanding and diagnosis of the disease.Methods Clinical data and MSCT findings of 5 cases with IgG4-related lung disease diagnosed by the pathological findings of operation or biopsy were analyzed retrospectively.Results 5 cases of IgG4-related lung diseases manifested as single nodule or mass in the lung.The size of lesions ranged from 17.6 mm× 13.6 mm× 49.5 mm to 51.8 mm× 38.0 mm× 37.7 mm.The lesion-lung interfaces were clear,with shallow lobulated edges and burr.5 cases grew around the bronchial and vessel.All 5 cases showed soft tissue density with patchy and/or strips of low density in plain scan,and slight to marked enhancement in enhanced scan.All 5 cases linked pleura with wide basement,and the fiber trabs around the lesions and enlargement of hilar and mediastinal lymph nodes were observed.Conclusion IgG4-related lung disease mainly present with intrapulmonary nodule or mass,with shallow lobulated edges and burr,and fiber trabs around the lesions.In lesions,the positive bronchial sign and vascular embedded sign can be seen,and the soft tissue density is homogeneous with slight to marked enhancement.
