T1ρ序列、MR和关节镜在膝关节软骨成像中的应用对比

【摘要】 目的 研究T序列对于膝关节骨关节炎(OA)的诊断价值,并对比T、MR与关节镜对早期髌骨软骨损伤诊断的敏感性。方法 共扫描28例住院患者的膝关节MRI,经后处理软件处理后,T序列轴位测量正常与损伤软骨的T值,所有病例均记录MR和关节镜结果。结果 T序列可以清晰地显示关节软骨,且呈现不同色阶,正常软骨T值均<50 ms,随着软骨损伤的加重T值逐渐增高,Ⅰ~Ⅱ级与Ⅲ~Ⅳ级软骨的数值差异有统计学意义。其中9例MR和关节镜下均为0级的软骨在T序列上出现T值升高,近似于Ⅰ~Ⅱ级软骨损伤,T序列对于早期软骨损伤的诊断敏感性高于MR与关节镜。另外,关节镜和MR对于软骨的诊断价值存在较高的一致性。结论 T序列能够无创地、有效地预测膝关节OA;T序列可先于MR与关节镜发现早期软骨损伤。

【Abstract】 Objective To explore the value of T in the diagnosis of knee osteoarthritis(OA),and to compare the diagnostic ability of T,MR and arthroscopy for early patellar cartilage injury.Methods 28 patients underwent T MR imaging.We processed the images and measured Ti p values of the normal and damage cartilage.All cases were recorded results of MR and arthroscopy.Results Tsequence could show cartilage clearly,and performed different colour levels,Tip values of normal cartilage were less than 50 ms,and increased with aggravation of cartilage injury.The differences of Ⅰ-Ⅱ and Ⅲ-Ⅳ had statistically significant.9 cases in grade 0 under MR and arthroscopy performed high values in T,which similar to grade Ⅰ-Ⅱ injury,the diagnosis sensitivity of T for early cartilage injury was higher than MR and arthroscopy.In addition,arthroscopy and MR had high consistency in the diagnosis of cartilage.Conclusion T can effectively predict early cartilage injury and knee OA non-invasively,and can detect early cartilageinjury before MR and arthroscopy.
