【摘要】 目的探讨晚期消化道肿瘤患者应用替吉奥胶囊治疗后的临床效果及对血管内皮生长因子(VEGF)和基质金属蛋白酶-9(MMP-9)表达的影响。方法 99例经病理检查证实的晚期食管癌、胃癌及大肠癌患者均给予单药替吉奥治疗,观察临床效果及不良反应。比较化疗前后外周血VEGF及MMP-9的表达情况。结果 99例患者化疗后完全缓解(CR)0例,部分缓解(PR)38例,病情稳定(SD)34例,病情进展(PD)27例,临床总缓解率38.3%,临床获益率72.7%。化疗的不良反应主要为Ⅰ~Ⅱ度骨髓抑制、消化道反应、肝功能损伤、手足综合征。化疗后外周血中VEGF及MMP-9含量明显下降,化疗前后表达差异有统计学意义(P<0.05);PR、SD、PD 3组患者外周血VEGF及MMP-9的表达差异有统计学意义(P<0.05)。结论晚期消化道肿瘤患者应用替吉奥胶囊治疗效果肯定,毒副作用小。血清测定VEGF及MMP-9的表达在一定程度上有助于判断预后,指导化疗。更多还原

【Abstract】 Objective To investigation the clinical efficacy of S-1 for advanced gastrointestinal cancer and its effect on serum vascular epithelial growth factor( VEGF) and matrix metalloproteinase-9( MMP-9) levels. Methods 99 cases of advanced gastrointestinal cancer patients confirmed by pathology were treated with S-1 chemotherapy. To observe the clinical effect and adverse reactions,and to compare before and after chemotherapy peripheral blood VEGF expression and the expression of MMP-9.Results 99 patients after chemotherapy,no case showed remission( CR),38 cases showed partial remission( PR),34 cases showed stable disease( SD),27 cases showed progressive disease( PD). The responsive rate( CR + PR) was 38. 3%,The clinical benefit rate( CR + PR + SD) was 72. 7%. The major adverse events were myelosuppression ranging from grade Ⅰ to Ⅱ,digestive tract reactions damage of liver function and hand-foot syndrome. VEGF and MMP-9 content in peripheral blood after chemotherapy significantly decreased,there had significant differences( P < 0. 05). PR,SD,PD patients peripheral blood expression of VEGF and MMP-9 had statistically significant difference( P < 0. 05). Conclusion S-1 shows positive effect for patients with advanced gastrointestinal cancer with small side effects.
