【摘要】 目的观察鼠神经生长因子联合自拟金铃通窍方治疗三叉神经痛术后患者的疗效。方法将收治的三叉神经痛患者94例按随机数字表法分为两组,每组47例,均实施微血管减压术,术后对照组不给予神经营养药物及中药治疗,观察组给予鼠神经生长因子加自拟金铃通窍方治疗。观察治疗前后疼痛、睡眠及中医证候积分变化,检测5-羟色胺(5-HT)、降钙素基因相关肽(CGRP)、白介素-1β(IL-1β)、血管活性肠肽(VIP)、β-内啡肽(β-EP)水平变化,统计治疗后6个月内复发情况。结果两组治疗后1、2周疼痛视觉模拟(VAS)评分比较差异无统计学意义(P> 0. 05),但治疗4周后观察组VAS评分低于对照组(P <0. 05)。观察组治疗1、2、4周匹兹堡睡眠质量指数表(PSQI)评分均低于对照组(P <0. 05)。观察组治疗后中医症状积分均低于对照组(P <0. 05)。观察组治疗后5-HT、β-EP高于对照组,而CGRP、VIP低于对照组(P <0. 05)。观察组治疗后6个月内复发率低于对照组(P <0. 05)。结论三叉神经痛患者术后给予鼠神经生长因子联合自拟金铃通窍方治疗,可有效缓解疼痛,提高患者生活质量,降低复发率。因此手术后辅助有效神经营养药物治疗三叉神经痛较单独手术治疗具有明显优势。更多还原

【Abstract】 Objective To observe the effects on trigeminal neuralgia treated with mouse nerve growth factor and the self-prepared jinling tongqiao formula. Methods A total of 94 patients of trigeminal neuralgia were divided into 2 groups according to random number table,47 cases in each one. All of the patients received the microvacular decompression. After operation,in the control group,no medication with western medicine and Chinese herbal medicine was used for neurotrophy. In the observation group,the combined therapy with mouse nerve growth factor and the self-prepared jinling tongqiao formula was provided. Before and after treatment,the pain,sleep and TCM symptom score were evaluated,the levels of 5-HT,CGRP,IL-1β,VIP and β-EP were determined. In 6 months of treatment,the recurrence was observed.Results In 1 and 2 weeks of treatment,the differences were not significant in VAS score between the two groups( P >0. 05). In 4 weeks of treatment,VAS score in the observation group was lower than the control group( P < 0. 05). In 1,2 and 4 weeks of treatment,PSQI scores in the observation group were lower than the control group( P < 0. 05). After treatment,the TCM symptom score in the observation group was lower than the control group( P < 0. 05). The levels of 5-HT and β-EP in the observation group were higher than the control group and those of CGRP and VIP were lower than the control group( P < 0. 05). The recurrence rate in the observation group was lower than the control group in 6 months of treatment( P < 0. 05). Conclusion After operation,the therapy with the mouse nerve growth factor and the self-prepared jinling tongqiao formula effectively relieves pain,improves the quality of life and reduces the recurrence rate. The accessory therapy with neurotrophy drugs after operation achieves the obvious advantages to the treatment of trigeminal neuralgia as compared with simple operation.更多还原
