【摘要】 目的闭合复位经皮螺钉内固定手术治疗跟骨骨折对患者踝关节、跟骨术后恢复的影响。方法选取我院骨科手术治疗的70例跟骨骨折患者,收治时间2014年1月至2016年2月,其中35例患者采用闭合复位经皮螺钉内固定手术治疗(研究组),对照组采用经皮"L"型切口切开内固定治疗(对照组),对比两组的手术相关指标、术后踝关节功能恢复情况、跟骨恢复指标。结果研究组手术时间、住院时间、住院费用均显著的低于对照组,差异有统计学意义(P<0. 05);研究组和对照组的骨折愈合时间比较,差异无统计学意义(P> 0. 05)。术后6个月复查,研究组踝关节功能优62. 86%、良31. 43%、可5. 71%,对照组优57. 14%、良28. 57%、可14. 29%,两组比较差异无统计学意义(P> 0. 05);术后1周、6个月,两组患者的Bolher角、跟骨高度较本组术前均显著的提高,两组患者的跟骨宽度较本组术前均显著的减小,差异有统计学意义(P<0. 05)。术前、术后1周及术后6个月,两组患者的Bolher角、跟骨高度、跟骨宽度组间比较,差异无统计学意义(P>0. 05)。结论闭合复位经皮螺钉内固定手术治疗跟骨骨折与切开复位内固定的效果基本一致,但具有手术创伤小、术后恢复快的优势。更多还原

【Abstract】 Objective To investigatethe influence of closed reduction and percutaneous screw internal fixation on the recovery of ankle and calcaneus in the treatment of calcaneal fracture.Methods 70 cases of calcaneal fracture receiving operation in orthopedics department of our hospital between January 2014 and February 2016 were selected as research objects.35 patients received closed reduction and percutaneous screw internal fixation( study group),while another 35 patients in the control group received open reduction and internal fixation with L-shaped incision.Operation-associated indicators,post-operation recovery of ankle function and recovery indicators of calcaneus of the two groups were compared. Results Operation time,hospitalization time and hospitalization cost in the study group were all significantly lower than those in the control group,the difference being statistically significant( P<0. 05).There was no statistically significant difference in fracture healing time between the study group and the control group( P>0. 05).Six months after operation,62. 86% of patients in the study group had grade A ankle function,with 31. 43% of grade B and 5. 71% of grade C,while 57. 14% of patients in the control group had grade A ankle function,with 28. 57% of grade B and 14. 29% of grade C,the differences being not statistically significant( P>0. 05).1 week and 6 months after surgery,the Bolher angle and calcaneal height of the two groups were significantly higher than those of the two groups before surgery,and the calcaneal width of the two groups was significantly shorter than that of the two groups before surgery,with statistically significant differences( P<0. 05).Before surgery,1 week after surgery and 6 months after surgery,there were no statistically significant differences between the two groups in Bolher angle,calcaneal height and calcaneal width( P>0. 05).Conclusion The effect of closed reduction and percutaneous screw internal fixation in the treatment of calcaneal fractures is basically the same as that of open reduction and internal fixation,but it has the advantage of little surgical trauma and quick postoperative recovery.更多还原
