下颌单颗后牙即刻种植负载后种植体周围龈沟液TNF-α、IL-1β、TGFα及OCN的变化及意义
Changes and Significance of TNF-α,IL-1β,TGFα and OCN in Gingival Crevicular Fluid around the Implant after Immediate Implantation of a Single Mandibular Posterior Tooth
【摘要】 目的本研究旨在研究下颌单颗后牙进行即刻种植负载后种植体周围龈沟液中TNF-α,IL-1β,TGFα,OCN的变化及意义,以期从分子水平比较种植体植入后周围组织随时间发生的微观变化。方法本研究共纳入40名患者(男22例,女18例),共植入40枚种植体,植入3d后对种植体进行负载,采集7、15、30、60、90d种植体周围龈沟液,并采用酶联免疫吸附法对其进行分析。结果 TNF-α、IL-1β在7d时高表达(P<0.05),随后缓慢下降。TGFα在15d时达到高峰(P<0.05),随后缓慢下降。OCN逐渐增高,15d后释放加快(P<0.05),在60d后维持高峰状态。但TNF-α,IL-1β,TGFα,OCN在60d后水平皆趋于稳定(P> 0.05)。结论 60d后种植体周围龈沟液中TNF-α,IL-1β,TGFα,OCN的浓度趋于平稳,表明机体已适应前期的负载,提示即刻负载相比于延期负载,在不影响种植体骨结合的情况下,可以缩短患者永久冠修复的时间。
【Abstract】 Objective This research was designed to analyze the changes and significance of TNF-α,IL-1β,TGFα and OCN in gingival crevicular fluid around the implant after immediate implantation of a single mandibular posterior tooth to compare the microscopic changes of surrounding tissue over time from the molecular level. Methods The participants included 40 patients( 22 men and 18 women). A total of 40 implants were implanted and they were unloaded after three days. The gingival sulcus around the implant was gathered in 7,15,30,60 and 90 days,using the method of enzyme-linked immunosorbent analysis. Results TNF-αand IL-1β were highly expressed at 7 days( P<0.05) and then decreased slowly. TGFα peaked at 15 days( P<0.05) and then decreased slowly. OCN gradually increased,and release accelerated after 15 days( P<0.05),and maintained a peak after 60 days.However,TNF-α,IL-1β,TGFα and OCN tend to be stable after 60 days( P>0.05). Conclusion TNF-α,IL-1β,TGFα and OCN tend to be stable after 60 days indicates that the body has adapted to the early load,indicating that the immediate load can shorten the time of permanent crown repair for patients without affecting the implant bone union compared with the delayed load.
