检测血清25-羟维生素D3在婴幼儿喘息中的应用价值
Application significance of serum 25-hydroxyl vitamin D3 in infants with wheezing
【摘要】 目的探讨血清25-羟维生素D3[25-(OH)D3]水平在喘息患儿的变化及临床意义。方法选择2016年10月至2017年4月湖州市中心医院收治的48例喘息及48例普通肺炎婴幼儿,同时选择健康儿童25例纳入对照组。比较不同研究对象血清25-(OH)D3及IgE检测结果,比较喘息及肺炎患儿维生素D缺乏及病毒检出情况,并比较不同维生素D水平喘息患儿的疗效指标。结果喘息组、肺炎组血清25-(OH)D3水平均明显低于对照组,血清IgE水平均高于对照组,差异均有统计学意义(q值分别为-45.99、-12.18、52.96、182.76,均P<0.01);喘息组血清25-(OH)D3水平低于肺炎组,血清IgE水平则高于肺炎组,差异均有统计学意义(q值分别为-40.85、156.83,均P<0.01);喘息组维生素D缺乏率、病毒阳性率明显高于肺炎组,差异均有统计学意义(χ2值分别为17.14、7.07,均P<0.05);喘息组维生素D充足患儿咳嗽、喘息及哮鸣音等消退时间、住院时间明显短于维生素D缺乏患儿,差异均有统计学意义(t值分别为2.15、2.68、2.14、2.54,均P<0.05)。结论维生素D与喘息的发生及发展关系密切,在临床诊治中应加强对患儿血清25-(OH)D3的检测,对低25-(OH)D3水平的婴幼儿应采取相应的治疗措施。
【Abstract】 Objective To investigate the changes and clinical significance of serum 25-hydroxyvitamin D3 [25-(OH)D3]in infants with wheezing.Methods Forty-eight cases of wheezing and 48 cases of common pneumonia treated in Zhejiang Huzhou Central Hospital from October 2016 to April 2017 were selected as subjects,and 25 cases of healthy infants were included in control group.The serum 25-(OH)D3 and IgE levels were compared among subjects,and vitamin D deficiency and virus detection in infants with wheezing and common pneumonia were compared.The efficacy of wheezing infants with different vitamin D levels was compared.Results The serum 25-(OH)D3 levels in the wheezing group and the pneumonia group were significantly lower but the serum IgE levels were higher than those in the control group(q value was-45.99,-12.18,52.96 and 182.76,respectively,all P<0.05).The serum 25-(OH)D3 level of the wheezing group was lower but the serum IgE level was higher than that in the pneumonia group with significant difference(q value was-40.85 and 156.83,respectively,both P <0.01).The incidence of vitamin D deficiency and virus positive rate in the wheezing group were significantly higher than those in the pneumonia group(χ2 value was 17.14 and 7.07,respectively,both P<0.05).In wheezing group the disappearance of cough,wheezing and wheezing rale,and the hospitalization time in cases with sufficient vitamin D were significantly shorter than those in cases with deficient vitamin D,and the differences were significant(t value was 2.15,2.68,2.14 and 2.54,respectively,all P<0.05).Conclusion Vitamin D is closely related to the occurrence and development of wheezing.In clinical diagnosis and treatment,the detection of 25-(OH)D3 should be strengthened,and corresponding treatment measures should be taken for infants with low 25-(OH)D3 level.
