MRI与CT诊断胫骨平台骨折患者的价值研究

【摘要】 目的观察分析MRI与CT诊断胫骨平台骨折(TPF)临床效果。方法选择2016年6月—2017年12月期间在本院进行TPF诊治的患者82例,均行MRI与CT检查,并比较MRI与CT检查结果与病理诊断结果。结果两种检测方式对骨折部位、骨折粉碎、骨折移位、骨缺损显示效果进行对比差异无统计学意义(t=0.2205、0.7612,、1.7452、1.1279,P>0.05);手术病理证实B1型21例、B2型15例、B3型9例、C1型22例、C2型9例、C3型6例; MRI检查B1、B2、B3、C2诊断符合率分别为100.00%、100.00%、100.00%、100.00%与CT检查分别为90.48%、93.33%、100.00%、88.89%(P>0.05),而MRI检查C1、C3诊断符合率分别为95.45%、100.00%显著地高于CT检查分别为81.82%、83.33%(P<0.05); MRI诊断骨折检出率为98.78%明显的高于CT诊断骨折检出率92.68%(χ2=3.5073,P<0.05),MRI诊断分型符合率为98.78%显著地高于CT诊断分型符合率(90.24%)(χ2=5.7606,P<0.05)。结论运用MRI与CT诊断胫骨平台骨折(TPF)均具有显著地诊断准确率,而MRI诊断TPF)具有更高的应用价值,值得临床大力推广应用。

【Abstract】 Objective To observe and evaluate the clinical effects of MRI and CT in the diagnosis of tibial plateau fracture( TPF).Methods Eighty-two patients who underwent TPF diagnosis and treatment in our hospital from June 2016 to December 2017 were all examined by MRI and CT. The results of MRI and CT examination and pathological diagnosis were compared.Results There were no significant differences in the effect of the two detection methods on fracture site,fracture comminution,fracture displacement,and bone defect display( t= 0.2205,0.7612,1.7452,1.1279,P>0.05).Surgical pathology confirmed 21 cases of type B1,15 cases of type B2,9 cases of type B3,22 cases of type C1,9 cases of type C2,and 6 cases of type C3.The diagnostic coincidence rates of MRI in the diagnosis of B1,B2,B3,and C2 were 100. 00%,100. 00%,100. 00%,and 100. 00%,respectively.The diagnostic coincidence rates of CT examinations were 90.48%,93.33%,100.00%,and 88.89%,respectively( P>0.05),while the diagnostic coincidence rates of C1 and C3 by MRI were 95.45.45% and 100.00%,which was higher than 81.82% and 83.33% of CT examination( P<0.05).The detection rate of MRI in the diagnosis of fracture( 98.78%) was significantly higher than 92.68% of the CT diagnosis( χ2= 3.5073,P<0.05),and the coincidence rate of MRI in diagnosis classification( 98.78%) was significantly higher than 90.24% of the CT diagnosis( χ2= 5. 7606,P < 0. 05). Conclusions The use of MRI and CT in the diagnosis of tibial plateau fractures( TPF) have significant diagnostic accuracy,while MRI has higher application value in diagnosis of TPF and is worthy of clinical application.
