规范化脑部控温策略对先天性心脏病体外循环术后婴幼儿脑氧饱和度的影响
The Effect of Standardized Brain Temperature Control Strategy on Cerebral Oxygen Saturation in Infants after Cardiopulmonary Bypass for Congenital Heart Disease
【摘要】 目的:通过评估规范化脑部控温策略对先天性心脏病术后婴幼儿脑氧饱和度的影响,探讨规范化脑部控温策略的临床意义。方法:将2016年8月~2017年9月复旦大学附属儿科医院CCU收治的94例先天性心脏病心脏畸形矫正术后患儿根据术后即刻实施脑保护策略的不同分为对照组和实验组,对照组采用单纯脑部降温策略,实验组采用规范化脑部控温策略;记录术后即刻及1~6、12、24、36、48 h脑氧饱和度数值。结果:两组术后即刻和36 h后脑氧饱和度比较差异无统计学差异(P> 0. 05);术后4~36 h时,实验组脑氧饱和度明显高于对照组(P <0. 05)。结论:给予先天性心脏病患儿术后规范化脑部控温策略对术后4~36 h内脑氧饱和度有明显改善作用。
【Abstract】 Objective: To explore the clinical significance of standardization of brain temperature control strategy by evaluating the influence of standardization temperature control strategy on brain oxygen saturation for congenital heart disease surgery infant. Method: The patients of 94 cases of children with congenital heart disease heart deformity correction surgery from Children’s Hospital of Fudan University CCU between August 2016 and September 2017 were divided into control group and experimental group based on immediate postoperative brain protection strategy. The control group adopted simple brain cooling strategy,and the standardized brain temperature control strategy was used for experimental group; The value of cerebral oxygen saturation was recorded immediately after surgery and 1 ~ 6,12,24,36,48 hours. Results: There was no statistically significant difference in cerebral oxygen saturation between the two groups immediately after surgery and 36 hours after surgery( P > 0. 05). The cerebral oxygen saturation in the experimental group was significantly higher than that in the control group after 4 ~ 36 hours of surgery( P < 0. 05). Conclusion: Standardized brain temperature control strategy for postoperative children with congenital heart disease can significantly improve cerebral oxygen saturation within 4 ~ 36 hours after surgery.
