颈动脉超声检查在缺血性脑血管疾病分型诊断中的应用价值研究

【摘要】 目的研究颈动脉超声检查在缺血性脑血管疾病分型诊断中的应用价值。方法选取2017年6月—2018年6月兰州大学第二医院接收并行颈动脉超声检查分型诊断的缺血性脑血管疾病患者57例作为研究组,并选取同期本院接收并体检的健康者55例作为健康对照组,两组均采用彩色多普勒超声诊断仪进行颈动脉超声检查,分析两组颈动脉内中膜厚度(IMT)、检出率及颈动脉粥样硬化血管斑块分布、分型情况。结果研究组的颈动脉IMT为(1.01±0.12)mm,低于健康对照组的(0.56±0.09)mm(t=22.389,P<0.001)。研究组颈动脉粥样硬化斑块总数、单发斑块、多发斑块所占比例均高于健康对照组,无斑块所占比例低于健康对照组(P<0.05)。两组颈动脉粥样硬化斑块分布情况比较,差异无统计学意义(χ2=0.052,P=0.974)。两组颈动脉粥样硬化斑块分型情况比较,差异无统计学意义(χ2=3.080,P=0.379)。结论颈动脉超声检查在缺血性脑血管疾病分型诊断中的应用价值显著,可推广。

【Abstract】 Objective To study the value of carotid ultrasonography in the classification of ischemic cerebrovascular diseases. Methods From June 2017 to June 2018, 57 patients with ischemic cerebrovascular disease diagnosed by carotid artery ultrasonography in the Second Hospital of Lanzhou University were selected as the study group, and 55 healthy persons received and examined in the same period were selected as the healthy control group. The carotid artery intima-media thickness(IMT),detection rate, distribution and classification of carotid atherosclerosis plaques were analyzed by color Doppler ultrasonography in both groups. Results The IMT of carotid artery in the study group was(1.01±0.12) mm, lower than that in the healthy control group(0.56±0.09) mm(t=22.389, P<0.001). The proportion of total carotid atherosclerotic plaque, single plaque and multiple plaque in the study group was higher than that in the healthy control group, and the proportion of non-plaque was lower than that in the healthy control group(P<0.05). There was no significant difference in the distribution of carotid atherosclerotic plaques between the two groups(χ2=0.052, P=0.974). There was no significant difference in carotid atherosclerotic plaque classification between the two groups(χ2=3.080, P=0.379). Conclusion Carotid ultrasonography is of great value in the classification and diagnosis of ischemic cerebrovascular diseases and can be popularized.
