【摘要】 目的探讨高频超声对高尿酸血症关节病变的早期诊断作用。方法在本院2016年1月至2017年5月检出的高尿酸血症患者中随机抽取95例为研究对象,根据是否有急性痛风发作分为无症状组52例,有症状组43例;另选90例健康体检者为对照组,均采用高频超声对双侧第一跖趾关节、膝关节、踝关节等进行观察。结果有症状组检出阳性率为67.44%,无症状组为38.46%,对照组未检出,3组差异有统计学意义(P<0.05)。有症状组共检出58个病变关节,其中第一跖趾关节病变47个,踝关节病变7个,膝关节病变4个;无症状组检出24个病变关节,均为第一跖趾关节病变。有症状组检出关节病变阳性患者29例,其中50岁及以下1例,51~60岁11例,60岁以上17例;无症状组检出关节病变阳性患者20例,其中50岁及以下1例,51~60岁5例,60岁以上14例。结论高频超声可检出无症状高尿酸血症患者关节病变,可作为筛选高尿酸血症患者的首选影像学检查方法,第一跖趾关节是重点筛查部位。高尿酸血症关节病变阳性率主要发生于50岁以上人群。更多还原

【Abstract】 Objective To investigate the role of high frequency ultrasound in the early diagnosis of joint lesions in patients with hyperuricemia.Methods A total of 95 cases with hyperuricemia in our hospital from January 2016 to May 2017 were randomly selected as the research objects and were divided into the asymptomatic group with 52 cases and the symptomatic group with 43 cases according to whether there was the acute gout attack,and another 90 healthy subjects were selected as the control group.The bilateral first metatarsophalangeal joint,knee joint and ankle joint of all patients were observed by high frequency ultrasound.Results The positive rate was 67.44% in the symptomatic group,38.46% in the asymptomatic group and not any in the control group.The differences between the three groups were statistically significant(P <0.05).A total of 58 joint lesions were detected in the symptomatic group,including 47 of the first metatarsophalangeal joint,7 of the ankle joint and 4 of the knee joint while 24 lesions were detected in the asymptomatic group,all of which were the first metatarsophalangeal joint.There were 29 cases with positive joint lesions in the symptomatic group,including 1 case less than 50 years of age,11 cases between 51 and 60 years old and 17 cases above 60 years old.There were 20 cases with positive joint lesions in the asymptomatic group,including 1 cases less than 50 years old,5 cases between 51 and 60 years old and 14 cases above 60 years old.Conclusion High frequency ultrasound can detect the joint lesions in patients with hyperuricemia,and it can be used as the first screening imaging method in patients with hyperuricemia.The first metatarsophalangeal joint is the key screening joint.The positive rate of joint lesions in patients with hyperuricemia mainly occurs in people over 50 years old.更多还原
