绝经后女性原发性高血压患者颈动脉斑块稳定性与LP-PLA2、A-FABP和hs-CRP的关系

【摘要】 目的探讨绝经后女性原发性高血压患者颈动脉斑块稳定性与脂蛋白相关磷脂酶A2(LPPLA2)、脂肪细胞型脂肪酸结合蛋白(A-FABP)和高敏C反应蛋白(hs-CRP)的相关性。方法选择2014年8月~2015年11月于青岛市第八人民医院住院的绝经后女性原发性高血压患者125例,年龄55~81岁,平均(65.98±7.47)岁。均行彩色多普勒超声检查确定有颈动脉斑块形成。根据斑块特征分为稳定斑块组(96例)和不稳定斑块组(29例)。收集所有患者一般临床资料与实验室检查结果(包括LP-PLA2、A-FABP等),颈动脉超声检测结果。结果不稳定斑块组的hs-CRP水平显著高于稳定斑块组,差异有统计学意义(P<0.05)。经多因素Logistic回归分析,绝经后女性原发性高血压患者hs-CRP(OR=2.162,95%CI:1.189~4.032)、A-FABP(OR=1.017,95%CI:1.001~1.038)是颈动脉斑块稳定性的影响因素。结论绝经后女性原发性高血压患者hs-CRP、A-FABP水平与颈动脉斑块的稳定性相关,LP-PLA2水平与颈动脉斑块稳定性无明显关系。

【Abstract】 Objective To discuss the correlation between stability of carotid plaques and levels of lipoprotein associated phospholipase A2(LP-PLA2), adipocyte fatty acid-binding protein(A-FABP) and high-sensitivity C-reactive protein(hs-CRP) in postmenopausal woman patients with essential hypertension(EH). Methods The postmenopausal woman patients with EH(n=125, aged from 55 to 81 and average age=65.98±7.47) were chosen from the Eighth People’s Hospital of Qingdao City from Aug. 2014 to Nov. 2015. All of them were given color Doppler ultrasound examination for diagnosing carotid plaque formation, and according to plaque features, they were divided into stable plaque group(n=96) and unstable plaque group(n=29). The general clinical materials, laboratory results(including LP-PLA2 and A-FABP), and carotid ultrasound results were collected from all patients. ResultsThe level of hs-CRP was significantly higher in unstable plaque group than that in stable plaque group(P<0.05). The results of multi-factor Logistic regression analysis showed that hs-CRP(OR=2.162, 95%CI: 1.189~4.032) and A-FABP(OR=1.017, 95%CI: 1.001~1.038) were influence factors on stability of carotid plaques in postmenopausal woman patients with EH. Conclusion The levels of hs-CRP and A-FABP is correlated to, and level of LP-PLA2 is not correlated to stability of carotid plaques in postmenopausal woman patients with EH.
