亚低温联合咪达唑仑治疗小儿重症病毒性脑炎临床研究
The effect of mild hypothermia combined with midazolam in the treatment of children with severe viral encephalitis
【摘要】 目的:探讨亚低温联合咪达唑仑对小儿重症病毒性脑炎的疗效。方法:选取小儿重症病毒性脑炎患儿60例,按随机数字表法分为观察组和对照组各30例。患儿均予以通畅呼吸道、脱水降低颅内压、抗感染等常规处理。观察组在此基础上入院时加用冰帽与冰毯的亚低温联合咪达唑仑治疗。于两组患儿入院时及入院后10 d时分别进行1~2 h脑电图监测评估。结果:观察组临床症状体征改善,病后3 d格拉斯哥昏迷量表(GCS)评分、入院第10 d脑电图的改善优于对照组,差异有统计学意义(P<0.05)。结论:亚低温联合咪达唑仑辅助治疗病毒性脑炎具有更好的脑保护作用,监测脑电图有助于临床治疗参考。
【Abstract】 Objective:To investigate the effect of mild hypothermia combined with midazolam in the treatment of children with severe viral encephalitis and observe the change of dynamic EEG. Methods :Sixty children with severe viral encephalitis were divided into the observation group(30 cases) and the control group(30 cases) by the random sampling table. Conventional treatment was conducted for all the cases including cleaning airway, reducing intracranial pressure by dehydration, anti-infection treatment and so on. On the basis of above processing the observation group was treated with mild hypothermia and midazolam with ice caps and ice blankets. EEG monitoring assessment was conducted for 1 ~2 hours in both groups respectively at the time of admission and 10 days after admission to the hospital Results:The clinical symptoms and vital signs of the observation group were improved, and according to the scores of Glasgow(GCS) and EEG 3 d after onset and 10 d the efficacy after admission were better than that for the control group(P<0. 05). Conclusion:Mild hypothermia combined with midazolam adjuvant treatment of viral encephalitis has better function of brain protection, EEG monitoring is helpful for clinical treatment of children with severe viral encephalitis.
