孕前体质量指数与后代先天性心脏病关联剂量反应关系的Meta分析
Association between Pre-pregnancy BMI and Congenital Heart Disease in Offspring: A Dose-response Meta-analysis
【摘要】 目的系统评估孕前体质量指数与后代先天性心脏病的关联。方法系统检索PubMed、EMbase、Web of Scicence、万方、维普和中国知网,检索时间为建库至2017年12月,按纳入和排除标准筛选文献。采用NOS量表评估文献质量。结果共16篇研究纳入本Meta分析,其中病例对照研究13篇,队列研究3篇。低体质量(OR:1.03,95%CI:0.97~1.10)和超重(OR:1.05,95%CI:0.98~1.14)与后代先天性心脏病的发生风险无关,肥胖(OR:1.37,95%CI:1.17~1.61)可增加后代先天性心脏病的发生风险。剂量反应的Meta分析提示两者间存在非线性剂量反应关系,即随着孕前体质量指数的增加,后代发生先天性心脏病的风险也随之增加。结论肥胖影响后代先天性心脏病的发生风险,孕前体质量指数越大,后代发生先天性心脏病的风险越高。
【Abstract】 Objective To systematically evaluate the association between pre-pregnancy BMI and congenital heart disease(CHD) in offspring. Methods We systematically searched Pubmed, EMbase, Web of Science, Wanfang, VIP and CNKI databases for eligible studies concerning the association between pre-pregnancy BMI and CHD in offspring from the inception date to December 2017 by strict inclusion and exclusion criteria. The NOS scale was used to assess the quality of literatures. Results Sixteen studies were included and 13 studies of them were case control studies, while the remaining cohort studies. The results revealed that underweight(OR: 1.03, 95%CI: 0.97~1.10) and overweight(OR: 1.05, 95%CI: 0.98~1.14) were not significantly associated with CHD in offspring,while obesity(OR:1.37, 95%CI: 1.17~1.61) was significantly associated with the increased risk of CHD in offspring. The dose response meta-analysis suggested that there was a nonlinear relationship between pre-pregnancy BMI and CHD in offspring. With the increasing pre-pregnancy BMI, the risk of CHD in offspring was increased. Conclusion Obesity is significantly associated with the risk of CHD in offspring. With the increasing pre-pregnancy BMI, the risk of CHD is increased.
