前列腺癌患者经直肠弹性成像技术和磁共振成像的临床诊断价值

【摘要】 目的 探讨经直肠超声弹性成像(TUE)和磁共振成像(MRI)对诊断前列腺癌(PCa)的临床应用价值。方法 选取有完整相关临床病史资料的患者45例,对患者行经直肠超声弹性成像及MRI检查,并与病理检查结果 进行比较。结果 45例患者经病理检查证实为前列腺癌28例。经直肠诊断符合率为73.33%,敏感性为78.57%,与MRI符合率75.56%、敏感性71.43%比较,差异无统计学意义(P>0.05),但其特异性(64.71%)低于MRI的特异性(82.35%),差异有统计学意义(P<0.05)。二者联合诊断符合率为91.11%,敏感性92.86%,特异性88.24%,与单独一种诊断比较,差异均有统计学意义(P<0.05)。癌组织平均应变比值(SR)为(27.45±34.10),良性病灶SR平均值为(2.87±0.54),差异有统计学意义。结论 TUE及MRI对前列腺癌的诊断各有优势,且TUE无痛无创,简单易行,其联合MRI可提高前列腺癌的检出率,对于诊断前列腺癌具有较高的临床价值。更多还原

【Abstract】 Objective To investigate the clinical value of transrectal ultrasound elastography and magnetic resonance imaging for the diagnosis of prostate cancer. Method Forty—five patients with complete relevant clinical history data were selected. The patients underwent transrectal ultrasound elastography and MRI and compared with the pathological findings. Result 45 patients were confirmed to have prostate cancer by pathological examination in 28 cases. The coincidence rate of rectal diagnosis was 73.33% and the sensitivity was 78.57%. The coincidence rate with MRI( 75.56%)and sensitivity( 71.43%) showed no significant diff-erence( P>0.05). But lower than the specificity of MRI( 82.35%), the difference was statistically significant( P<0.05). The coincidence rate of the two methods was 91.11%, the sensitivity was 92.86% and the specificity was 88.24%. There was significant difference between the two methods(P<0.05). Strain ratio( SR) of cancer tissue was( 27.45 ± 34.10) in average, and( 2.87 ± 0.54)in benign lesion. The difference was statistically significant. Conclusion Transrectal UE and MRI have their own advantages in the diagnosis of prostate cancer, and the transrectal UE is painless and noninvasive and simple. The combination of MRI and MRI can improve the detection rate of prostate cancer and has high clinical value for the diagnosis of prostate cancer.更多还原
