健康成人气管结构与身高体重及体质量指数相关性的CT研究

【摘要】 目的 对正常成年人气管支气管内径进行测量并计算气管横截面积,分析气管支气管内径、横截面积与身高、体重及体质量指数的相关性,为临床术前气管插管提供参考。方法 100例正常成年男性和100例正常成年女性进行胸部CT扫描,利用CT三维后处理软件测量主动脉弓上缘1cm处气管内径(横径和前后径)及左右主支气管开口处内径,计算气管横截面积,分析其与身高、体重及BMI的相关性。结果 男性主动脉弓上缘1cm处气管横径和前后径分别为(17.39±1.87) mm和(18.61±1.76) mm,女性主动脉弓上缘1cm气管横径和前后径分别为(15.45±1.70)mm和(15.50±1.75)mm,男性左、右主支气管开口处内径为(14.26±1.53)mm和(15.68±1.55)mm,女性左、右主支气管开口处内径为(12.78±1.60) mm和(14.09±1.35) mm,男女性气管横截面积分别为(256.33±38.24) mm~2和(189.04±35.41)mm~2。男性身高与横截面积有一定的相关性(r=0.337, P=0.001),其余男性体重(r=0.229, P=0.022)及BMI(r=0.069, P=0.496),女性身高(r=0.194, P=0.053)、体重(r=-0.016, P=0.875)及BMI (r=-0.018, P=0.242)与横截面积无明显相关性。结论 男性气管主支气管内径及横截面积均大于女性,男女性右主支气管内径大于左主支气管内径,男性的身高与气管横截面积有一定的相关性,男性身高可以作为选择气管插管时的一个参考依据,建议利用CT后处理技术确定气管大小,以此来选择合适的气管插管。

【Abstract】 Objective To measure the diameter of the trachea and bronchus of normal adults and calculate the cross-sectional area of the trachea, and to evaluate the relationship between the BMI of adults and the diameter and cross-sectional area of trachea in order to provide the reference for the preoperative intubation. Methods 100 males and 100 females underwent chest CT were selected, the diameter of trachea at aortic arch superior border horizontal 1 cm and the main bronchus were measured at the workstation. Results The transverse and anteroposterior trachea diameters( TD and APD) of the Men’s were( 17.39 ± 1.87) mm and( 18.61 ± 1.76) mm respectively. The transverse and anteroposterior diameters of the female were(15.45± 1.70) mm and( 15.50± 1.75) mm. The diameter of the men’s left and right main bronchus( LBD and RBD) were( 14.26± 1.53) mm and( 15.68 ± 1.55) mm, the diameter of the women’s left and right main bronchus were( 12.78 ± 1.60) mm and( 14.09 ± 1.35) mm. The crosssectional area of male and female trachea were(256.33 ± 38.24) mm2 and( 189.04 ± 35.41) mm2. There was significant difference between male height and cross-sectional area(r=0.337, P=0.001), but there was no significant difference between cross-sectional area and weight( male: r=0.229,P=0.022 female: r=-0.016, P=0.875), BMI(male: r=0.069, P=0.496 female: r=-0.018, P=0.242), female height(r=0.194, P=0.053).Conclusion The men’s trachea, main bronchus diameter and tracheal cross-sectional area are greater than women. The diameter of the right main bronchus of male and female is greater than that of the left main bronchus. There is significant difference between male height and cross-sectional area.Male height can be used as a reference for selecting endotracheal tubes( ETT). We propose that three-dimensional reconstruction can be used to determine the size of the trachea to select the appropriate ETTs.
