缺血修饰白蛋白对甲状腺功能减退患者的诊断价值
【摘要】 目的 探讨缺血修饰白蛋白(IMA)在甲状腺功能减退症(甲减)患者中的诊断价值。方法 检测158例甲减患者白蛋白(Alb),缺血修饰白蛋白(IMA),IMA/Alb,总胆固醇(TC),甘油三酯(TG),高密度脂蛋白胆固醇(HDL-C)等指标,并与80例甲状腺功能正常者(对照组)比较。结果 亚临床甲状腺功能减退症(SHT)患者IMA浓度及IMA/Alb值与对照组比较,差异有统计学意义(P<0.05);临床甲状腺功能减退症(OHT)患者IMA浓度及IMA/Alb值与SHT患者比较,差异有统计学意义(P<0.05);且对照组、SHT患者、OHT患者IMA浓度及IMA/Alb值均呈递增式正相关关系。在OHT组患者中,促甲状腺激素(TSH)浓度与IMA浓度(r=0.518,P<0.01)及IMA/Alb值(r=0.509,P<0.05)呈显著正相关关系;而甲状腺素(T4)浓度与IMA浓度(r=-0.364,P<0.05)呈显著负相关关系。但在SHT组患者中,各个变量间未发现显著相关关系。结论 甲减患者体内随着抗氧化力的减少,IMA的浓度会增加,这表明甲减患者体内存在氧化应激,并在OHT患者中显得尤为突出。因此,IMA可以作为甲减患者体内存在氧化应激的标志物,不但可辅助诊断甲减,且能鉴别甲减的类型以区分OHT和SHT。
【Abstract】 Objective To aid the diagnosis of hypothyroidism and distinguish overt hypothyroidism( OHT) and subclinical hypothyroidism(SHT) through the detection of the patients of ischemia-modified albumin levels. Methods Albumin( Alb),ischemia-modified albumin(IMA), IMA/Alb, total cholesterol( TC), triglyceride( TG), high density lipoprotein cholesterol( HDL-C) indicators, etc were detected in 158 patients with hypothyroidism, and 80 euthyroid controls were also tested these indicators. Results The IMA concentration and IMA/Alb value of SHT patients compared with the control group, P<0.05. The IMA concentration and IMA/Alb value of OHT patients compared with SHT patients,P<0.05; and the IMA concentration and IMA/Alb value of these three groups was incrementally positive correlation.In patients with OHT group,thyroid stimulating hormone(TSH) concentration and IMA( r=0.518, P<0.01) and IMA/Alb( r=0.509, P<0.05) correlation analysis found a significant positive correlation; yet thyroxine( T4) concentration and IMA( r=-0.364, P<0.05) there was a significant negative correlation. But in patients with SHT group, no significant correlation was found between the various variables. Conclusion With the reduction in antioxidant capacity of in patients with hypothyroidism, the concentration of IMA will increase, which indicates that there is a body of a patient with hypothyroidism oxidative stress, and in OHT patients is particularly prominent. Thus, IMA can be used as marker of oxidative stress in patients with hypothyroidism, not only can be used to aid diagnosis of hypothyroidism, and can be used to identify the type of hypothyroidism to distinguish between OHT and SHT.
