【摘要】 目的:初步建立北京市社区高血压长处方患者慢病管理的药学服务模式并对效果进行评价。方法:随机选取北京市通州区张家湾社区高血压患者,其中长处方组50例,非长处方组49例,对长处方组患者定期进行高血压相关知识及用药教育培训、定期随访、用药指导、药物咨询等药学服务。将药学服务前后患者对高血压知识的认知水平、血压达标情况,用药依从性等进行统计分析。结果:药学服务后,与非长处方组比较,患者对高血压疾病和治疗知识的认识水平有明显提高(P <0.05),患者血压控制水平及用药依从性明显改善(P<0.05)。结论:开展社区高血压长处方患者慢病管理的药学服务,可以明显提高患者对疾病的认知水平、治疗效果和依从性,从而提高患者的生活质量,值得推广。更多还原

【Abstract】 Objective: To establish a pharmaceutical service model for chronic diseases management of patients with hypertension who are prescribed with medications for 2 months(long-term prescription) in communities in Beijing and evaluate the effectiveness. Methods : Hypertensive patients in Zhangjiawan community, Tongzhou District, Beijing were randomly selected Among them, 50 patients were assigned to the long-term prescription group and 49 to the non-long-term prescription group.The patients in the long-term prescription group were provided with pharmaceutical services such as education on hypertension and medication related knowledge and regular follow-up and drug counseling. Before and after the pharmaceutical service, the patients’ cognition of hypertension, blood pressure control rate, and medication compliance were statistically analyzed Results:After pharmaceutical care, compared with non-long prescription group, the patients’ knowledge of hypertension and treatment was significantly increased(P<0.05), and the blood pressure control rate and medication compliance were significantly improved(P<0.06). Conclusion: The pharmaceutical service for the management of chronic diseases in hypertension patients in communities with long prescription can obviously improve the patients’ cognition level, treatment effectiveness and compliance, and can improve the patients ’quality of life.
