【摘要】 目的探究全髋关节置换术对股骨颈骨折老年患者术后髋关节功能及并发症发生率的影响。方法选取2015年3月~2017年2月襄城县人民医院股骨颈骨折老年患者82例,按照手术方法分为两组,每组各41例。对照组行半髋关节置换术,观察组行全髋关节置换术。对比两组临床疗效、手术情况及并发症发生率,采用髋关节功能(Harris)评分评价手术前后两组髋关节功能。结果观察组手术总有效率高于对照组,差异有统计学意义(P<0.05);观察组手术时长及拔管时间长于对照组,手术出血量高于对照组,差异有统计学意义(P<0.05);两组并发症发生率无显著差异(P>0.05);术后观察组Harris评分高于对照组,差异有统计学意义(P<0.05)。结论全髋关节置换术治疗股骨颈骨折老年患者,虽手术情况较差,但疗效显著,术后髋关节功能恢复良好,值得临床应用。更多还原

【Abstract】 Objective To investigate the effect of total hip arthroplasty on postoperative hip function and complication rate in elderly patients with femoral neck fracture. Methods 82 elderly patients with femoral neck fracture in Xiangcheng County People’s Hospital from March 2015 to February 2017 were divided into 2 groups according to the operation methods,41 cases in each group. The control group received hemiarthroplasty, and the observation group received total hip replacement. The clinical efficacy, operation and complication rate were compared between the two groups. The hip function(Harris) score was used to evaluate the hip function of the two groups before and after operation. Results The total effective rate of the observation group was higher than that of the control group(P < 0.05). The operation time and extubation time in the observation group were longer than those in the control group, and the volumn of bleeding in the operation group was higher than that in the control group(P < 0.05).There was no significant difference in the incidence of complications between the two groups(P > 0.05). The Harris score of the observation group was higher than that of the control group(P < 0.05). Conclusion The operation of total hip replacement for elderly patients with femoral neck fracture is poor, but the curative effect is remarkable, and the postoperative hip function recovery is good.
