闭环式信息管理系统对临床输血质量影响分析

【摘要】 目的:通过建立并实施闭环式输血信息管理系统,实现血液全过程跟踪管理,观察其对临床输血质量的影响。方法:实施闭环管理后2013-05—2015-04患者为观察组,实施前为对照组,分析2组患者一般情况、红细胞输注情况、不良反应发生率及不同适应证分组输血前后血红蛋白变化和红细胞输注有效率。结果:(1)观察组和对照组平均红细胞输注量(U)分别为4.82±0.98、5.35±1.32,比较差异有统计学意义,不良反应发生率分别为3.72%、1.67%,差异有统计学意义(P<0.05);(2)观察组和对照组输血前血红蛋白值(g/L)分别为74.53±21.68、78.22±22.59,比较差异有统计学意义(P<0.05);(3)各适应证分组间比较,输血前血红蛋白值除急性失血组比较差异无统计学意义,其余各组差异均有统计学意义;输血后血红蛋白值各组比较差异均无统计学意义;(4)红细胞输注有效率观察组显著高于对照组(72.7%vs 67.3%,χ2=96.87;P<0.05)。结论:闭环式输血信息管理系统可以对输血全过程进行监控,把握最佳的输血适应证,有效节约血液资源,提高输血疗效,及时发现不良反应,保证患者输血安全。

【Abstract】 Objective:To observe the effect of close-loop blood transfusion information management system(CL-BIMS)on the quality of clinical blood transfusion.Method:Data from patients with red blood cell(RBC)transfusion from the Affiliated Hospital of Qingdao University were collected.The patients were classified as control group from May,2011 to April,2013 before the application of CL-BIMS.The ones were classified as observation group from May,2013 to April 2015 after the application of CL-BIMS.The general conditions,RBC transfusion,adverse effects rate,the variation of hemoglobin(Hb)before and after transfusion in different groups with different transfusion indications and the transfusion effective rate were analyzed.Result:The mean transfusion amount of RBCs was(4.82±0.98)U and(5.35±1.32)U in observation group and control group,respectively.There was significant difference between the two groups.The transfusion adverse effects rate was 3.72% and 1.67%in the two groups(P<0.05),respectively.There was significant difference between them.The Hb level was(74.53±21.68)g/L and(78.22±22.59)g/L in observation group and control group before transfusion,respectively.The difference was significant between the two groups(P<0.05).The Hb level was(99.59±21.45)g/L and(101.39±24.17)g/L after transfusion,respectively.The difference was not significant between them(P<0.05).There was significant difference among different groups with different transfusion indication except the acute blood loss group before transfusion.After transfusion,there was no significant difference among the groups.The transfusion effective rate in observation group was significantly higher than that in control group(72.7% vs67.3%,χ2=96.872,P<0.01).Conclusion:CL-BIMS could monitor the whole process of blood transfusion,save the blood effectively,improve the efficiency of blood transfusion,detect the adverse reactions early and benefit to patient safety.
