【摘要】 目的探讨经阴道子宫输卵管四维超声造影(TVS 4D-HyCoSy)在评价输卵管妊娠保守性手术后输卵管通畅性中的临床应用价值。方法对200例行输卵管妊娠保守性手术的患者均在术后3个月采用TVS 4D-HyCoSy检查,分析所获得图像,评估其输卵管通畅性。其中64例患者均在TVS 4D-HyCoSy检查后2个月采用腹腔镜直视下输卵管通液术检查,并分析TVS 4D-HyCoSy检查与腹腔镜直视下输卵管通液术检查的通畅性符合率。结果200例患者中,双侧通畅20例,双侧通而不畅17例,双侧阻塞16例,一侧通畅一侧阻塞80例,一侧通畅一侧通而不畅53例,一侧通而不畅一侧阻塞14例。64例患者中,行128条输卵管检查,TVS 4D-HyCoSy显影的128条中与腹腔镜下通液术检查结果符合116条,不符合12条。TVS 4D-HyCoSy检查的输卵管通畅、通而不畅和阻塞与腹腔镜下通液术检查的符合率分别为92.00%(46/50)、81.82%(18/22)、92.86%(52/56)。结论 TVS 4D-HyCoSy可准确评价输卵管妊娠保守性手术后输卵管通畅性。更多还原

【Abstract】 Objective To explore the clinical value of transvaginal four-dimensional contrastenhanced ultrasound(TVS 4 D-HyCoSy)in the evaluation of tubal patency after conservative surgery for tubal pregnancy.Methods TVS 4 D-HyCoSy was performed in 200 patients to assess the tubal patency 3 months after conservative surgery for tubal pregnancy.Among them,64 patients underwent laparoscopic hydrotubation 2 months after TVS 4 D-HyCoSy examination.The compliance rate of TVS 4 D-HyCoSy and laparoscopic examination was analyzed.Results Among the200 cases,20 were bilaterally unobstructed,17 were bilaterally unobstructed but unsmooth,16 were bilaterally obstructed,80 were unobstructed on one side and obstructed on the other side,53 were unobstructed on one side and unsmooth on the other side,and 14 were obstructed on one side and unsmooth on the other side.Among the 64 patients receiving laparoscopic hydrotubation(128 fallopian tubes),the TVS 4 D-HyCoSy was consistent with laparoscopic hydrotubation in 116 fallopian tubes,which did not meet 12 fallopian tubes.The compliance rates for tubal patency,unobstruction with obstacle and obstruction were 92.00%(46/50),81.82%(18/22)and 92.86%(52/56),respectively.Conclusion TVS 4 D-HyCoSy can accurately evaluate the tubal patency after conservative surgery for tubal pregnancy.更多还原
