胃肠肿瘤患者术后切口感染情况及高危因素分析
Analysis of incidence and high risk factors of postoperative incision infection in patients with gastrointestinal cancer
【摘要】 目的 探讨胃肠道肿瘤患者术后切口感染情况及其危险因素,为临床防治提供依据。方法 对214例胃肠道肿瘤病患者术后发生切口感染的情况进行调查,并对患者的临床资料、手术季节、住院时间、肿瘤类型、体质量指数、合并症种类、切口性质、是否放置引流管及是否预防性使用抗菌药物等情况进行单因素及多因素logistic回归分析。结果 本组胃肠道肿瘤患者术后切口感染率为12.15%。单因素及多因素分析显示,年龄≥60岁、住院时间≥14 d、手术时间≥3 h,合并症≥3种、夏季手术、体质量指数≥35 kg·m~2、切口长度≥15 cm及预防使用抗菌药物是影响术后切口感染的独立危险因素(P<0.05)。结论 胃肠道肿瘤患者术后易发生切口感染,其影响因素众多,临床应结合患者实际情况予以有针对性的干预,降低术后感染率,促进患者早日康复。
【Abstract】 Objective To investigate the postoperative incision infection status and its risk factors in patients with gastrointestinal cancer, and to provide evidence for clinical prevention and treatment. Methods The incidence of wound infection in 214 patients with gastrointestinal cancer were investigate. The basic clinical data, season of surgery, time of hospitalization, type of tumor, body mass index(BMI),type of complication, nature of surgical incision, placement of drainage tube and whether the use of antimicrobial agents were investigated and taken for univariate and multivariate logistic regression analysis. Results The postoperative incision infection rate of patients with gastrointestinal cancer in this group was 12. 15%.Univariate and multivariate analysis showed that, age ≥ 60 years, hospitalization time ≥ 14 days, operation time ≥ 3 h, complication ≥ 3, summer surgery, body mass index ≥ 35 kg · m~2, incision length15 cm and application of antibiotics were independent factors affecting postoperative wound infection(P<0. 05). Conclusion Patients with gastrointestinal tumors are prone to wound infection after operation.There are many influencing factors and the clinical should be combined with the actual situation of the patient to provide targeted interventions to reduce the postoperative infection rate and promote the patient’s early recovery.
