阿替普酶联合头部亚低温治疗早期急性脑梗死临床研究

【摘要】 目的 探讨阿替普酶联合头部亚低温治疗对早期急性脑梗死患者神经功能缺损程度及血清心型脂肪酸结合蛋白、S100β蛋白水平的影响。方法 将106例早期急性脑梗死患者按抽签法分为两组,各53例。两组均采取阿替普酶治疗,观察组在此基础上联合头部亚低温治疗。比较两组治疗效果、不良反应发生情况、神经功能缺损程度、日常生活能力及血清心型脂肪酸结合蛋白、S100β蛋白水平。结果 治疗后观察组总有效率显著高于对照组(P<0.05)。治疗前两组各量表评分比较差异无统计学意义(P>0.05);治疗后观察组美国国立卫生研究院卒中量表评分显著低于对照组(P<0.01),简化巴氏指数量表评分显著高于对照组(P<0.01),血清心型脂肪酸结合蛋白、S100β蛋白水平显著低于对照组(P<0.01)。两组不良反应发生率比较差异无统计学意义(P>0.05)。结论 阿替普酶联合头部亚低温治疗早期急性脑梗死疗效显著,可有效降低血清心型脂肪酸结合蛋白、S100β蛋白水平,缓解患者神经功能损伤症状,提高其日常生活能力及生活质量。

【Abstract】 Objective To investigate the effects of alteplase combined with head mild hypothermia on neurological impairment and serum H-FABP and S100 beta protein levels in patients with early acute cerebral infarction. Methods 106 patients with acute cerebral infarction were divided into two groups according to the lottery method, each with 53 cases. The two groups were all treated with alteplase, and the observation group was plus mild hypothermia on this basis. The treatment effects, occurrence of adverse reactions,degree of neurological deficits,daily living ability,serum heart-type fatty acid-binding protein,and S100β protein levels in the two groups were compared. Results After treatment, the total effective rate of the observation group was significantly higher than that of the control group(P<0. 05). There was no significant difference in the scale scores of the two groups before treatment(P>0. 05). After treatment,the NIHSS score of the observation group was significantly lower than that of the control group(P<0. 01), the Barthel Index score was significantly higher than that of the control group(P<0. 01), and the levels of serum heart-type fatty acid binding protein and S100β protein were significantly lower than those of the control group(P<0. 01). There was no significant difference in the incidence of adverse reactions between the two groups(P>0. 05). Conclusion Alteplase combined with head mild hypothermia therapy is effective for early acute cerebral infarction patients. It can effectively reduce the serum H-FABP and S100 beta protein levels, relieve the symptoms of neurological impairment, improve their daily living ability and quality of life.
