磁共振脂肪定量分析在运动 节食治疗非酒精性脂肪肝中的应用
Analysis of the effect of two different weight loss methods of exercise and diet on nonalcoholic fatty liver by magnetic resonance quantitative analysis
【摘要】 目的 探究磁共振成像技术中非对称回波的最小二程估算法迭代水脂分离技术定量分析对运动、节食治疗非酒精性脂肪肝患者临床疗效的评估价值。方法 将40例非酒精性脂肪肝患者按随机数字表法分为两组,每组20例,研究组行运动治疗,对照组行节食治疗,观察3个月。于治疗前及治疗3个月测定患者肝功能指标、血脂指标、体质量指数,观察两组疗效,并通过磁共振成像技术中非对称回波的最小二程估算法迭代水脂分离技术测定肝脏脂肪分量值,对肝脏脂肪分量值与血脂指标进行相关分析。结果 治疗前两组各指标比较差异无统计学意义(P>0.05);治疗后研究组谷草转氨酶、谷丙转氨酶、总胆红素水平均较治疗前显著下降(P<0.01),且各项指标水平显著低于对照组(P<0.01);研究组肝脏脂肪分量值、体质量指数及三酰甘油水平较治疗前显著下降(P<0.05或0.01),肝脏脂肪分量值、体质量指数、胆固醇、三酰甘油水平低于对照组,高密度脂蛋白胆固醇水平及总有效率高于对照组,但两组间比较差异无统计学意义(P>0.05)。磁共振肝脏脂肪分量值与胆固醇、三酰甘油水平呈显著正相关(r=0.814、0.809,P<0.05),与高密度脂蛋白胆固醇水平呈显著负相关(r=-0.784,P<0.05)。结论 运动、节食治疗非酒精性脂肪肝均有一定效果,运动治疗对患者肝功能及血脂水平的改善更为显著,优于节食治疗;磁共振成像技术中非对称回波的最小二程估算法迭代水脂分离技术定量分析可为运动、节食治疗非酒精性脂肪肝的效果评估提供一定依据。
【Abstract】 Objective To investigate the value of IDEAL-IQ in the evaluation of weight loss methods of exercise and diet on nonalcoholic fatty liver by magnetic resonance quantitative analysis. Methods 40 cases with NAFLD were randomly divided into two groups, 20 cases in each group. The observation group underwent exercise therapy,the control group underwent diet therapy, patients were observed for 3 months. The liver function indexes, blood lipid indexes, BMI of the two groups before treatment and at 3 months after treatment were measured, clinical effects were observed, the liver fat values were detected by IDEAL-IQ, the relation between the liver fat values and blood lipid indexes were analyzed. Results Before treatment, there was no statistic significance in each index between the two groups(P>0. 05); after treatment, AST, ALT, TBil levels in the observation group were significantly declined compared with those before treatment(P<0. 01), each index level was significantly lower than that in the control group(P<0. 01); the liver lipid value, BMI, triacylglycerol level in the observation group were significantly declined compared with those before treatment(P<0. 05 or 0. 01), the liver lipid value, BMI, cholesterol,triacylglycerol level were lower than those in the control group, HDL level and total effective rate were higher than those in the control group, but the difference was not statistic significant(P>0. 05). The liver F value of MRI was positively correlated with the levels of TC and TG(r = 0. 814, 0. 809, P<0. 05), and negatively correlated with the level of HDL-C(r =-0.784, P<0. 05). Conclusion Exercise, diet therapy in the treatment of nonalcoholic fatty liver disease have a certain effect, exercise treatment is better than dieting on the improvement of liver function and serum lipid level, the technology of quantitative magnetic resonance IDEAL-IQ analysis can provide a basis for exercise, diet therapy in the treatment of nonalcoholic fatty liver.
