【摘要】 目的报道2例硬脑膜动静脉瘘(DAVF)导致的双侧丘脑病变病例。方法收集2例经数字减影血管造影(DSA)明确的DAVF导致的双侧丘脑病变患者的临床和影像学资料,结合文献复习进行分析。结果病例1,男性,63岁,因反应迟钝、懒言少动近2个月就诊,头颅MRI见双侧丘脑异常信号。DSA见直窦区域DAVF,行动静脉栓塞术,每年随访反应迟钝依然存在,但生活基本能自理。病例2,男性,63岁,因记忆力下降、反应迟钝1个月收治入院。头颅MRI见直窦内少许条状等信号,考虑直窦血栓,伴大脑深静脉梗死可能,继发左侧丘脑出血。DSA见直窦-窦汇区DAVF,予瘘口栓塞术,术后2个月随访,生活自理能力已完全恢复。结论 DAVF导致丘脑病变临床少见,起病隐匿,临床表现或影像学表现相似的疾病较多,诊断困难。遇有进行性认知功能下降,且影像学检查有双侧丘脑病变伴有肿胀出血者,应考虑DAVF可能。更多还原

【Abstract】 Aim To report two cases of bilateral involvement of thalamus caused by dural arteriovenous fistula(DAVF).Methods The clinical and radiological data of two cases of bilateral involvement of thalamus caused by dural DAVF confirmed by digital substracted angiography(DSA) was collected.Results Case 1 was a 63 year old male with 2 month long languor and obtuseness,MRI revealed bilateral thalamus lesions and DSA revealed DAVF near the straight sinus.Transarterial embolization was performed.His activity of daily living was normal though obtuseness remained.Case 2 was a 63 year old male complained of declined memory and obtuseness for one month,MRI revealed abnormal signals in bilateral thalamus and straight sinus.DSA revealed DAVF near the straight sinus.Transarterial embolization was performed and he completely recovered after 2 months.Conclusion Bilateral thalamus lesions caused by DAVF was a rare condition,which occurs insidiously and causes devastating symptoms,and may be easily misdiagnosed as other diseases.更多还原
