【摘要】 目的探讨应用Angiojet血栓抽吸系统治疗急性下肢深静脉血栓(acute lower extremity deep venous thrombosis, LEDVT)的临床效果。方法回顾性分析2016年5月至2016年10月收治的17例LEDVT患者临床资料。患者经下腔静脉滤器(inferior vena cava filter, IVCF)植入、Angiojet血栓抽吸系统行血栓抽吸、导管接触性溶栓(catheter-directed thrombolysis, CDT),一期或后期对髂静脉压迫综合征进行球囊扩张及支架植入术(percutaneous trans—luminal angioplasty and stenting, PTAS)治疗。结果 17例患者一期血栓清除率80%～100%。17例患者中13例患者合并髂静脉压迫综合征,7例患者一期行经皮腔内血管成形术与支架置入术(percutaneous transluminal angioplasty, PTA)或PTAS。术后CDT置管时间平均(5.2±1.3)天,较单纯CDT置管时间[平均(7.3±1.8)天]缩短。结论 Angiojet血栓抽吸系统能够安全、迅速、有效清除急性下肢深静脉血栓,联合CDT治疗能够更彻底清除血栓,缩短溶栓时间,是治疗急性LEDVT的新型有效方法。更多还原

【Abstract】 Objective To investigate the clinical effect of the therapy for acute lower extremity deep venous thrombosis(acute lower extremity deep venous thrombosis, LEDVT) with angiojet thrombectomy System. Methods Clinical data of 17 admitted patients who underwent angiojet thrombectomy from May 2016 through October 2016 were analysed. All the patients underwent Angiojet Thrombectomy and catheter-directed thrombolysis(catheter-directed thrombolysis, CDT) after placement of inferior vena cava filter(inferior vena cava filter, IVCF). Patients with iliac vein stenosis or occlusion underwent venoplasty and stent implanted. Results Thrombus clearance of 17 patients was 80%～100% in one stage procedure. 13 patients were diagnosed as iliac vein compression syndrome, 7 of them underwent percut aneoustransluminalangioplasty and stent implanted in one-stage operation. The mean treatment duration for thrombectomy and CDT was5.2±1.3 dcompared with 7.3±1.8 d for CDT alone. Conclusion Angiojet Thrombectomy System is emerging as a safe, effective therapy for LEDVT which can remove thrombus as soon as possible, clear more thrombus combined with CDT.更多还原
