【摘要】 目的 :分析总结扁平苔藓(LP)的发病年龄、临床表现、分型及实验室检查结果。方法 :回顾性分析该院178例LP患者的临床资料。结果:178例LP患者平均发病年龄(46.7±17.1)岁,男女比例1.0∶1.3,50~59岁发病率最高(37.1%)。其中慢性局限性LP(48.3%)最常见,其次为线状LP(16.3%)、急性泛发性LP(5.6%)、光线性LP(5.1%)、大疱性LP(5.1%)、环状LP(4.5%)、色素性LP(3.4%)、口腔LP(2.8%)、毛囊性LP(2.8%)、肥厚性LP(2.2%)、LP-红斑狼疮重叠综合征(2.2%)、类天疱疮样LP(1.1%)及掌跖LP(0.6%)。38例(21.3%)伴口腔受累。所有患者抗核抗体(ANA)阳性率为32.6%,其中LP-红斑狼疮重叠综合征阳性率最高。雷公藤多苷治疗对皮肤型LP有效。结论:LP临床表现差异较大,慢性局限性LP是最常见亚型。LP-红斑狼疮重叠综合征患者ANA阳性率最高,其他亚型与ANA关系有待进一步探讨。更多还原

【Abstract】 Objective: To analyze the epidemiological, clinical and laboratory features of lichen planus(LP). Methods: The clinical features of LP were retrospectively analyzed in 178 patients. Results: The mean age of onset was 46.7±17.1 years, with a male to female ratio of 1.0∶1.3. The highest incidence was found in subjects aged 50 to 59 years old(37.1%). Chronic localized lichen planus was the most common subtype(48.3%), followed by linear lichen planus(16.3%), acute generalized lichen planus(5.6%), lichen planus actinicus(5.1%), bullous lichen planus(5.1%), lichen planus annularis(4.5%), lichen planus pigmentosus(3.4%), oral lichen planus(2.8%), follicular lichen planus(2.8%), lichen planus hypertrophicus(2.2%), overlap syndrome of lichen planus-lupus erythematosus(2.2%), pemphigoid-like lichen planus(1.1%) and lichen planus of the palms and soles(0.6%).Involvement of oral mucosa was found in 38 patients. 32.6% of patients displayed anti-nuclear antibody(ANA) positive. Patients with overlap syndrome of lichen planus-lupus erythematosus exhibited the highest positive rates of ANA. Tripterygium glycosides was effective for cutaneous type LP. Conclusions: The clinical manifestations of LP vary greatly. The most common subtype of LP is chronic localized LP. Patients with overlap syndrome of lichen planus-lupus erythematosus display the highest ANA positive rate.Further studies are required to delineate the relationships between other subtypes of LP and ANA.更多还原
