【摘要】 目的研究抑郁症患者心理防御机制及无抽搐电休克(MECT)治疗前后的变化。方法将64例抑郁症住院患者,采用随机数字表法分为实验组33例和对照组31例。两组按照常规治疗,实验组在此基础上联合MECT治疗。观察6周,于治疗前后采用DSQ问卷自评表对患者心理状态进行评估;采用焦虑自评量表(SAS)、抑郁自评量表(SDS)评定焦虑、抑郁情绪。采用两组问卷因子变量间线性相关系数多元逐步回归的方法分析抑郁与心理防御方式的相关性。结果实验组MECT治疗后不成熟防御机制因子、中间型防御制因子评分高于对照组(P<0.05),而成熟防御机制因子评分低于对照组(P<0.05)。两组治疗前SAS、SDS评分比较差异无统计学意义(P>0.05),实验组治疗后SAS、SDS评分分别为(35.28±4.42)分和(36.73±5.03)分,均低于对照组(P<0.05)。实验组治疗总有效率为90.9%,明显高于对照组83.9%(P<0.05)。SDS总评分与不成熟防御机制因子、中间型防御机制因子评分呈正相关,而与成熟防御机制因子呈负相关。结论抑郁症患者MECT治疗前后心理防御机制使用由不成熟心理防御机制向成熟心理防御机制上发展,有利于临床治疗。更多还原

【Abstract】 Objective To study the psychological defense mechanism of depressive patients and the difference before and after MECT.Methods Sixty-four depression hospitalized patients,were divided into experience group(33 cases)and control group(31 cases).Conventional therapy was applied in both groups,and MECT was used in experience group.After six weeks’ observation,DSQ self-assessment table was applied to evaluate patients’ mentality before and after treatments.SAS and SDS were used to evaluate patients’ anxiety and depression.Linear(pearson)correlation coefficient multivariate stepwise regression between two groups of factor variables was applied to analyse relevance between depression and Psychological Defense Pattern.Results Scores of immature defense mechanism before MECT in experience group were higher than that of control group(P<0.05),and scores of mature defense mechanism were lower than control group.The difference had statistical sense.Scores of anxiety depression before treatment between two groups had no statistical significance(P>0.05).Scores of anxiety depression were(35.28±4.42)and(36.73±5.03)after treating,which were both lower than control groups,difference was statistically significant(P<0.05).Conclusion Psychological Defense Mechanism of depressive patients developed from immature defense mechanism to mature defense mechanism before and after MECT,which had benefits in clinic treatment.更多还原
