【摘要】 [目的]对比分析腹腔镜与开腹右半结肠癌根治术的术后并发症发生率,并探讨腹腔镜右半结肠癌术后并发症发生原因及影响因素。[方法]回顾性分析河南大学淮河医院胃肠外科2010年01月至2017年01月由同一组医师收治的右半结肠癌患者521例的临床资料,其中观察组348例接受腹腔镜右半结肠癌根治手术,对照组173例接受常规开腹右半结肠癌根治术。比较2组患者术后2周内手术并发症的发生情况,分析腹腔镜右半结肠癌根治术患者并发症的发生原因及影响因素。[结果]腹腔镜组患者术后并发症发生率与开腹组相比,差异无统计学意义(P=0.95> 0.05),其中患者年龄、术前合并症、手术时间等是右半结肠癌术后并发症发生的影响因素(均P <0.05)。[结论]腹腔镜右半结肠癌根治术并不增加术后并发症的发生率,其术后并发症的发生与多种因素相关,应结合患者的个体情况制定相应的预防和干预策略。更多还原

【Abstract】 [Objective]Analyzing the postoperative complication rates between the groups of laparoscopic and open radical resection of right colon cancer comparatively, and exploring the causes and influencing factors of laparoscopic radical resection of right colon cancer.[Methods]The clinical data of 521 patients of right colon cancer who had received right semi-colectomy that operated by the same group of surgeons in the gastrointestinal department of Huaihe hospital of Henan university from January2010 to January 2017 were analyzed retrospectively, the group of laparoscopic right semi-colectomy included 348 cases, and the control group of open right semi-colectomy included 173 cases, the complication rates within 2 weeks after surgery between two groups were compared, and the causes and influencing factors of laparoscopic right semi-colectomy complications were analyzed.[Results]There were no significant difference between the laparoscopic and open groups in the postoperative complication rates(P= 0.95 > 0.05), the influencing factors of postoperative complications of laparoscopic right semi-colectomy included age,complications of preoperation, operation time etc( P < 0.05).[Conclusion]The postoperative complication rates of laparoscopic radical resection of right colon cancer did not increase significantly, which associated with a variety of factors, the prevention and intervention strategies should be institued according to the individual situations.更多还原
