用间期荧光原位杂交技术检测人慢性粒细胞白血病微小残留病的研究

【摘要】 [目的]探讨间期荧光原位杂交技术检测慢性粒细胞白血病(CML)骨髓和外周血细胞的bcr-abl融合基因的临床价值,并和常规染色体分析方法相比较。[方法]应用FISH技术和常规染色体分析技术对CML患者及正常对照组骨髓和外周血细胞bcr-abl融合基因进行分析。[结果]FISH检测对照组骨髓和外周血细胞阳性率为0.8%~3.7%,其临床分界值为2.67%,CML慢性期患者骨髓和外周血细胞bcr-abl阳性率分别为(59.9±24.6)%和(66.4±21.9)%,加速期分别为(76.2±17.9)%和(84.8±13.2)%,急变期分别为(78.3±20.1)%和(79.5±19.9)%,两者和CML慢性期相比差异具有明显的统计学意义(P <0.05)。Pearson相关系数分析发现,外周血细胞和骨髓细胞融合基因bcr-abl阳性率呈现典型的线性关系,同时发现在临床症状缓解的患者中,经骨髓移植的CML患者较药物治疗患者有较高的分子生物学缓解率(P <0.05)。[结论]间期荧光原位杂交技术(FISH)较常规染色体分析技术(CC)可特异性的检测CML患者治疗前后微残留状态,具有较高的准确性、可靠性、灵敏性。

【Abstract】 [Objective]Comparing with the conventional chromosome analysis method, to explore the detection value on chronic myelogenous leukemia by interphase fluorescence in situ hybridization.[Methods ]FISH technology and the conventional chromosome analysis technique was applied to detect the expression of bcr-abl fusion gene of the bone marrow and peripheral blood cells of the CML patients.[Results]The positive rate of the control group bone marrow and peripheral blood cell is 0.8% ~3.7%, its clinical bondary value is 2.67%, the positively expression rate of bcr-abl fusion in peripheral and bone marrow cell was( 59.9± 24. 6) % and( 66. 4 ± 21.9) % of the patients with chronic phase, were 76. 2 ± 17. 9) % and( 84. 8 ± 13. 2) % in those with accelerating phase, and were( 78.3± 20.1) % and( 79.5± 19.9) % in those with blastic crisis, comparing with the CML chronic phase,the difference of the accelerating phase and the blastic crisis phase has obvious statistical significance( P < 0.05), the perason correlation coefficient analysis found that the expression of the fusion gene bcr-abl of the peripheral blood cells and bone marrow cell presents typical liner relationship. Moreover, the bone marrow transplant patients have higher remission rate than other groups( P < 0.05).[Conclusion]the interphase fluresence in situ hybrization( FISH) can detect bcr-abl fusion gene of CML patients after the treatment, and it has the high accuracy, reliability and sensitivity.
