增强CT联合脱落细胞检测在甘肃会宁地区宫颈癌预防筛查中的应用

【摘要】 目的探讨增强CT联合脱落细胞检测在甘肃会宁地区宫颈癌预防筛查中的应用。方法回顾性分析2013年5月至2017年12月期间,在我院接受诊治的259例宫颈疾病的女性患者,均经宫颈刮片脱落细胞检测及增强CT扫描检查,分析两种检测异常者病理诊断结果及宫颈病变情况,统计两种检测方法对宫颈癌诊断的检出率。结果 259例会宁籍女性宫颈疾病患者,经脱落细胞检查显示无上皮内病变或恶性病变161例(62.16%)和结果异常者98例(37.84%);对于脱落细胞检查结果异常的98例患者,行增强CT扫描检查检出宫颈癌35例(检出率13.51%),癌前病变63例(检出率24.32%);脱落细胞检查结果异常的98例患者行宫颈活检,共检出宫颈癌39例(检出率15.06%),癌前病变50例(检出率19.31%)。脱落细胞检测联合增强CT在宫颈癌筛查中的异常率、癌前病变检出率、宫颈癌检出率与病理检查结果相比差异无统计学意义(P>0.05)。结论脱落细胞检测联合增强CT扫描可快速明确患者疾病,与临床病理检查检出率相当,有利于患者后期治疗和预后。

【Abstract】 Objective To investigate the applicability of enhanced CT combined with exfoliated cell detection in the prevention and screening of cervical cancer in Huining,Gansu Province. Methods The clinical data on 259 women with cervical diseases treated in our hospital between May 2013 and December 2017 was retrospectively analyzed. All these patients were examined by a scraper for cervical exfoliated cells and enhanced CT scan. The pathological diagnosis and cervical lesions in patients with abnormal detection were analyzed. The sensitivity,specificity,positive predictive value and negative predictive value of the two methods for the diagnosis of cervical cancer were statistically analyzed. Results Among the 259 patients with cervical disease in Huining,161 cases( 62. 16%) showed no intraepithelial lesions or malignant lesions,and 98 cases with abnormal results were found( 37. 84%,98/259). Among the 98 patients with abnormal results of exfoliative cell examination,there were 35 cases of cervical cancer( a detection rate of 13. 51%),and 63 cases of precancerous lesions( a detection rate of 24. 32%) detected by enhanced CT scanning. Cervical biopsy was performed in these 98 patients. A total of 39 cases of cervical cancer were detected( 15. 06%),and 50 cases were precancerous lesions( 19. 31%). The abnormal rate,detection rate of precancerous lesions,and the positive rate of cervical cancer detected with exfoliated cells combined with enhanced CT during cervical cancer screening were not significantly different from those of pathological examinations( P>0. 05). Conclusion Exfoliated cell detection combined with enhanced CT scan can quickly identify the disease,which can match the detection rate of clinicopathological examination.This approach can facilitate subsequent treatment of patients and help prevent the occurrence of cervical cancer.
