糖皮质激素对急性肺损伤小鼠肺功能影响研究

【摘要】 目的探讨糖皮质激素对急性肺损伤小鼠肺功能的影响。方法选取清洁级雄性BALB/C小鼠18只,随机分为3组,每组6只:对照组尾静脉注射生理盐水0. 1 ml,脂多糖组尾静脉注射脂多糖5. 0 mg/kg,激素组在脂多糖组基础上给予甲强龙4. 8 mg/kg。在模型建立6、18、36 h后,检测肺功能。结果 6 h后,甲强龙组气道压力较脂多糖组下降; 18 h后,甲强龙组气道压力较脂多糖组升高,两组比较,差异均有统计学意义(P <0. 05)。36 h后,甲强龙组肺容积、气道阻力均较脂多糖组升高,差异有统计学意义(P <0. 05)。6 h后,甲强龙组最大吸气流量较脂多糖组下降,差异有统计学意义(P <0. 05)。18、36 h后,甲强龙组2秒末用力呼气容积、最大呼气中期流速、用力最大呼气流速均较脂多糖组升高,最大呼气中期时间较脂多糖组下降,差异均有统计学意义(P <0. 05)。结论糖皮质激素可通过延缓气道压力升高,增加肺容积、气道阻力、2秒末用力呼气容积、最大呼气中期流速、用力最大呼气流速,减少最大吸气流量、最大呼气中期时间,改善急性肺损伤小鼠的肺功能。

【Abstract】 Objective To investigate the effect of glucocorticoid on mouse pulmonary dysfunction in the process of acute lung injury.Methods A total of 18 clean grade male BALB/C mice were selected and randomly divided into 3 groups,with 6 mice in each group:0. 1 ml of normal saline was injected into the tail vein of the control group,5. 0 mg/kg was injected into the tail vein of the lipopolysaccharide group,and 4. 8 mg/kg was given to the hormone group on the basis of the lipopolysaccharide group. Lung function was measured 6,18 and 36 hours after model establishment. Results After 6 hours,the airway pressure of methylprednisolone group was lower than that of lipopolysaccharide group. After 18 hours,the airway pressure of methylprednisolone group was higher than that of lipopolysaccharide group. The difference between the two groups was statistically significant( P < 0. 05). After 36 hours,lung volume and airway resistance in the methylprednisolone group were higher than those in the lipopolysaccharide group,and the difference was statistically significant( P < 0. 05). After 6 hours,the maximal aspirate flow in the methylprednisolone group was lower than that in the lipopolysaccharide group,and the difference was statistically significant( P < 0. 05). After 18 and 36 hours,the forced expiratory volume at the end of 2 seconds in the methylprednisolone group,the maximum expiratory flow velocity at the middle of the breath and the maximum expiratory flow velocity at the middle of the breath were higher than those in the lipopolysaccharide group,and the maximum expiratory time at the middle of the breath was lower than that in the lipopolysaccharide group,and the differences were statistically significant( P < 0. 05). Conclusion Glucocorticoid can improve the lung function of acute lung injury mice by delaying the increase of airway pressure,increasing lung volume,airwayresistance,forced expiratory volume at the end of 2 seconds,maximum mid-expiratory flow rate,maximum mid-expiratory flow rate,reducing the maximum inspiratory flow rate and the maximum mid-expiratory time.
