【摘要】 目的探讨新鲜周期D3选择性单胚胎移植的围产结局。方法回顾性研究2014年1月-2016年12月年龄<38岁在我院生殖中心予常规IVF/ICSI助孕治疗后移植新鲜D3优质胚胎的的患者。按移植胚胎数分为单优胚移植组(A组,470例)与双优胚移植组(B组,4545例)。分析比较两组的临床妊娠结局及产科分娩结局。结果 A组临床妊娠率、活产率均较B组显著降低(52.98% vs 72.78%,41.28%vs 62.95%,P<0.001);而A组胚胎种植率与B组相比无统计学差异(52.98%vs 54.60%,P>0.05);A组较B组多胎妊娠率显著下降(3.61% vs 43.83%,P<0.001)。A组早产率和早期早产率分别为7.22%和0%,均显著低于B组23.59%和2.48%(P<0.001,P=0.026)。A组低体重儿出生率和极低体重儿出生率分别为6.50%和0%,均显著低于B组29.92%和2.71%(P<0.001,P=0.018)。结论 D3选择性单胚胎新鲜移植具有与双胚胎移植相似的胚胎种植率,而且可以明显降低多胎妊娠率,同时可以获得更好的产科分娩结局。更多还原

【Abstract】 Objective To explore perinatal outcome of selective D3 single embryo fresh transfer. Methods A retrospective study was conducted to assess clinical pregnancy outcomes and obstetric outcomes of D3 high quality embryo fresh transfer cycles with female age<38 after conventional IVF/ICSI assisted reproduction treatment in our center between January 2014 and December2016. According to the number of embyo transfer,the cycles were divided into two groups,including single good quality embyo transfer group(A group,470 cycles) and double good quality embryos transfer group(B group,4545 cycles). Results Clinical pregnancy rate and live birth rate of A group were significantly lower than those of B group(52.98% vs 72.78%,41.28% vs 62.95%,P<0.001). There was no statistics difference on embyo implantation rate between A and B group(52.98% vs 54.60%,P >0.05).Multiple pregnancy rate of A group was significantly lower than that of B group(3.61% vs 43.83%,P<0.001). Preterm birth rate and early preterm birth rate of A group were significantly lower than those of B group(7.22% vs 23.59%,P<0.001;0% vs 2.48%,P=0.026). Low birth weight rate and very low birth weight rate of A group were significantly lower than those of B group(6.50% vs29.92%,P<0.001;0% vs 2.71%,P=0.018). Conclusion Selective D3 single embryo fresh transfer can cause the similar embyo implantation rate and the significantly lower multiple pregnancy rate compared with double embyo transfer.
