OLYMPUS生化分析仪检测尿mALB时后带现象预警参数设置的研究

【摘要】 目的通过对免疫比浊法测定尿微量白蛋白(mALB)的反应曲线的研究,在奥林帕斯自动生化分析仪设置尿m ALB检测的预警参数,报警后带现象,提示检验人员处理。方法在确定免疫比浊法检测mALB的线性范围和检测的最大稀释倍数前提下,通过对一系列浓度梯度mALB样品进行检测,分析不同浓度mALB样品的时间-吸光度反应曲线,最终吸光度反应曲线,比较不同浓度mALB样品在检测点限定时间段内吸光度变化值的差值,选择合适的后带现象预警参数。结果采用厂家声明的线性范围300μg/L,最大稀释倍数为50倍,时间-吸光度反应曲线的拐点浓度为1848.68μg/L。一系列标本浓度中,标本浓度超过340μg/L的时间-吸光度曲线明显发生了偏移。选择15读点到11读点差值与27读点到11读点差值的比值>0.6000,并且27读点到11读点差值>0.0716作为抗原过量的判断标准。结论通过正确的预警参数设置,可有效避免尿mALB检测过程中由于后带效应造成的结果偏差。

【Abstract】 Objective To set check parameters in Olympus automatic biochemical analyzer,in order to monitor post-band phenomenon in immunoturbidimetry detection of urinary microalbumin (mALB). Methods The time-absorbance reaction curves of different concentrations of mALB samples were analyzed to estemate the linear range of mALB and the maximum dilution factor of the immunoturbidimetric assay. The difference in absorbance change values of different concentrations of mALB samples in the defined time period of the detection point,and the appropriate early-band phenomenon warning parameters were selected. Results The linear range of the manufacturer was stated to be 300μg/L,the maximum dilution factor was 50 times,and the inflection point concentration of the time-absorbance reaction curve was 1848.68μg/L. In a series of specimen concentrations,the time-absorbance curve of the specimen concentration exceeding 340μg/L was significantly shifted. The ratio of the difference between reading point 15 to reading point 11 and the difference between reading point 27 to reading point 11 was selected as >0.6000,and the difference between reading point 27 to reading point 11 >0.0716 is used as a criterion for determining the antigen excess. Conclusion It is only through setting the correct check parameter that the deviation of the results due to the post band effect during the urine m ALB detection process is avoided.
