【摘要】 目的:探讨呼出气一氧化氮测定对门诊慢性咳嗽诊断和治疗的意义。方法:选取90例慢性咳嗽患者为研究对象,对其实施支气管激发试验并测定呼出气一氧化氮,同时对结果进行分析。结果:嗜酸粒细胞性支气管炎40例,咳嗽变异性哮喘30例,其他慢性咳嗽20例,三组患者一氧化氮值对比,差异有统计学意义(P <0. 05);三组患者测定的第1秒呼气容积(FEV1%)、用力肺活量(FVC%)、呼气峰流速(PEF)对比,差异有统计学意义(P <0. 05);咳嗽变异性哮喘的敏感度达到73. 33%(22/30),特异度则为93. 33%(28/30);嗜酸粒细胞性支气管炎的敏感度达到66. 67%(20/30),特异度则为90. 00%(27/30)。90例患者呼出气一氧化氮检测后有效率明显高于检测前,差异有统计学意义(χ2=16. 000,P=0. 000)。检测后不良反应率明显低于检测前差异有统计学意义(χ2=9. 944,P=0. 002)。结论:慢性咳嗽病因不同则一氧化氮水平值变化也不同,即可以通过一氧化氮的检测结果来诊断病因,进而采取更为有效的治疗方式,从而大大促进了慢性咳嗽的临床治疗。更多还原

【Abstract】 Objective To investigate the exhaled nitric oxide determination of the diagnosis and treatment of chronic cough in clinic. Method 90 patients with chronic cough were selected as the research objects. The bronchial provocation test and exhaled nitric oxide were tested,and the results were analyzed. Results Eosinophilic bronchitis 40 cases,30 cases of cough variant asthma and other chronic cough in 20 cases,three groups of patients with nitric oxide value contrast differences( P < 0. 05); FEV1%,FVC% and PEF were significantly different between the three groups( P < 0. 05); cough variant asthma sensitivity reached73. 33%( 22/30),the specificity was 93. 33%( 2/30); eosinophil bronchitis sensitivity reached 66. 67%( 20/30),the specificity was 90. 00%( 27/30). The effective rate of exhaled nitric oxide in 90 patients was significantly higher than that before test,and the difference was statistically significant( χ2= 16. 000,P = 0. 000). Conclusion The etiology of different levels of nitric oxide in chronic cough variation is different,that is to say,the detection results of nitric oxide to diagnose the cause,and take more effective treatment,which greatly promoted the clinical treatment of chronic cough.更多还原
