血清CD146与缺血性卒中后认知障碍的相关性研究

【摘要】 目的:探讨血清CD146与缺血性卒中后认知障碍的相关性。方法:收集缺血性脑卒中病患118例作为病例组,以健康体检的52例体检者为对照组,搜集入选者的病历资料等。病例组观察随访3个月,使用蒙特利尔认知评估量表(Mo CA)测评,并根据其结果分为有认知障碍64例和无认知障碍54例。采用ELISA法测血清CD146,比较各组血清CD146的差异;采用Spearman分析CD146与Mo CA值的关联性;使用Logistic回归统计方法分析并寻找卒中后认知障碍的危险因素,分析血清CD146与缺血性卒中后认知障碍的关系。结果:(1)病例组于卒中后3个月行Mo CA评估,认知障碍发病率为54.42%;(2)病例组CD146高于对照组[(8.17±2.56)pg/ml vs(7.76±1.12)pg/ml],差异有显著统计学意义(t=-9.420,P<0.01);无认知障碍组CD146高于有认知障碍组[(10.45±1.72)pg/ml vs(6.24±1.21)pg/ml],差异有显著统计学意义(t=13.087,P<0.01);(3)Spearman关联性分析CD146与Mo CA值呈正相关(r=0.725,P<0.01);(4)多因素Logistic回归分析CD146与缺血性卒中后认知障碍存在回归关系(OR=0.040,P<0.01)。结论:(1)低CD146浓度是缺血性卒中后认知障碍的独立危险因素;(2)CD146浓度越低,认知功能损害越严重,CD146是缺血性卒中后认知障碍的保护因素。

【Abstract】 Objective To explore the correlation of serum CD146 and cognitive impairment after ischemic stroke.Method 118 cases of ischemic stroke were selected as the case group and in our hospital physical examination center 52 cases as a control group,the clinical data of each group was collected.The case group was followed up for 3 months.According to the Montreal Cognitive Assessment(Mo CA) score,they were divided into 64 cases of cognitive impairment group,54 cases without cognitive impairment group.The concentration of serum CD146 were determinated by ELISA and compared the differences of serum CD146 concentration of each group;The correlation between serum CD146 level and cognitive impairment was analyzed using logistic regression analysis and to analysis the correlation of serum CD146 and cognitive impairment after ischemic stroke.Results(1) The case group was evaluated by Mo CA 3 months after stroke,the incidence of cognitive impairment was54.42%.(2) Compared with control group,serum CD146 level increased in patients with ischemic stroke [(8.17 ± 2.56) pg/ml vs(7.76 ± 1.12) pg/ml](t=-9.420,P<0.01);Compared with impaired cognition group,serum CD146 level increased in normal cognition group [(10.45 ± 1.72) pg/ml vs(6.24 ± 1.21) pg/ml](t=13.087,P<0.01).(3) Spearman correlation analysis showed a positive correlation between serum CD146 levels Mo CA score(r=0.725,P<0.01).(4) Multivariate logistic regression analysis CD146(OR=0.040,P<0.01) There was a regression relationship with cognitive impairment after stroke.Conclusion(1) Low serum CD146 concentration is an independent risk factor for post-ischemic stroke cognitive impairment.(2) The lower the concentration of CD146,the more severe cognitive impairment,CD146 is a protective factor of cognitive impairment after ischemic stroke.
