显微手术联合钉棒内固定治疗椎管退行性疾病的疗效观察
Observation of culative effect of microsurgical decompression of spinal canal combined with screw-rod internal fixation for the treatment of spinal canal degenerative diseases
【摘要】 目的:探讨椎管退行性疾病患者采取显微手术联合钉棒内固定治疗的临床效果。方法:将62例椎管退行性疾病患者分成两组。采取常规开放椎管减压联合螺钉内固定术为对照组,显微手术联合螺钉内固定术为试验组。将两组患者手术情况(切口长度、手术时间、术中出血量、住院天数)与临床效果进行比较。术后1周内及术后第1个月、第3个月、第6个月、第18个月时应用Oswestry功能障碍指数(ODI)评分法对患者进行评定。结果:试验组在术中出血量、术后引流液量明显少于对照组,差异有统计学意义(P<0.05),两组切口长度、手术时间、住院天数等比较,差异无统计学意义(P>0.05)。两组末次随访时ODI与术前比较,差异均有统计学意义(P<0.05),两组间术前及术后ODI对比,差异无统计学意义(P>0.05)。结论:对于椎管退行性疾病患者采取显微手术,同时联合螺钉内固定治疗,可有效减少出血量,改善症状,缩短住院恢复时间,临床治疗效果理想,值得临床推广。
【Abstract】 Objective To observe the effect of microsurgery combined with screw-rod internal fixation in the treatment of patients with spinal canal degenerative diseases. Method 62 cases of spinal degenerative diseases were divided into two groups according to the order of admission in our hospital. Routine open spinal canal decompression combined with screw internal fixation was used as control group,and microsurgery and internal fixation with screws were used as experimental group. The operation condition( incision length,operative time,intraoperative blood loss,length of stay) between the two groups were compared with the clinical effect. ODI score was used to evaluate the patients within 1 weeks,first months,third months,sixth months and eighteeth months after operation. Results Compared with the control group,the experimental group was significantly less than the control group in the amount of bleeding,the amount of drainage,the difference was statistically significant( P < 0. 05),and there was not significantly difference in the length of incision,the time of operation and the days of hospitalization between the two groups( P > 0. 05). ODI and preoperative comparison of the two groups at the last follow-up( P < 0. 05). ODI at the final follow-up had no significant differences between the two groups( P > 0. 05). Conclusion The treatment of spinal canal degenerative disease patients with microsurgery combined with screw-rod internal fixation can effectively decrease the amount of bleeding,shorten the hospital recovery time,improve the effect of clinical treatment,and value the clinical popularization.
