老年2型糖尿病(T2DM)并亚临床甲状腺功能减退症患者促甲状腺激素(TSH)水平对左心室功能的影响

【摘要】 目的:主要研究老年2型糖尿病(T2DM)合并亚临床甲状腺机能减退症患者TSH(促甲状腺激素)水平对左心室功能的影响。方法:选取99例老年T2DM并亚临床甲状腺机能减退症患者为观察组,选取同一时期99例老年T2DM未合并亚临床甲状腺机能减退症患者为对照组,评价老年T2DM并亚临床甲状腺机能减退症患者TSH水平对患者左心室功能的影响。结果:观察组患者的血脂水平[胆固醇(TC)、三酰甘油(TG)]与对照组比较,差异有统计学意义(P<0.05)。观察组患者TSH水平>10 m U/L者LDL与TC水平明显增高,其左心室收缩与舒张功能明显下降;而患者TSH水平5~10 m U/L时上述指标改变不明显,组间数据相比,差异有统计学意义(P<0.05)。结论:老年T2DM并亚临床甲状腺机能减退症患者TSH水平较高时,会引发患者左心室功能异常,存在危及患者生命安全的风险,但是TSH水平5~10 m U/L时对患者左心室功能影响不明显,因此在临床上应区别对待。

【Abstract】 Objective To study the effect of TSH level on left ventricular function in elderly patients with T2 DM combined with subclinical hypothyroidism. Method 99 elderly patients with type 2 diabetes and subclinical hypothyroidism treated in our hospital were selected as the observation group. At the same time,99 elderly patients with type 2 diabetes without subclinical hypothyroidism were selected as the control group. The effect of TSH level on left ventricular function in elderly patients with T2 DM and subclinical hypothyroidism was evaluated. Results Compared with the control group,the serum lipid levels of TC and TG in the observation group were significantly different from those of the control group( P < 0. 05). it was found that the levels of LDL and TC in the observation group were significantly higher than those of the 10 m U/L,and the left ventricular systolic and diastolic function decreased significantly,while the changes of the above indexes in the observation group were not obvious when the observation group was TSH 5 to 10 m U/L,and the data between the groups were compared with those of the group( P < 0. 05). Conclusion the high level of TSH in patients with T2 DM and subclinical hypothyroidism may lead to abnormal left ventricular function and risk the life safety of patients,but the effect of TSH 5 to 10 m U/L on left ventricular function is not obvious,so it should be treated differently in clinical.
