自动选择最佳期相在心律不齐患者冠状动脉CTA中的应用

【摘要】 目的 :探讨自动选择最佳时相功能在心率不齐患者冠状动脉CTA中的应用。方法 :对32例心律不齐患者行CTA检查,采用Autogating推荐的时相区间行单心动周期扫描。分别以中心期相及自动选择最佳期相重建图像。测量2种重建图像主动脉弓根部的CT值和噪声值,并计算其SNR,由2名经验丰富的CT医师对重建图像的主血管以5分制进行主观评分。采用秩和检验比较2组图像的SNR和质量评分。结果:2种重建图像主动脉弓根部的SNR比较差异有统计学意义(P<0.05)。中心期相右冠状动脉、左冠状动脉主干、左回旋支及左前降支的图像质量评分与对应的自动选择最佳期相评分比较差异均有统计学意义(均P<0.05)。结论 :心律不齐患者行冠脉CTA检查时,自动选择最佳时相重建血管图像可获得良好的图像质量以满足临床诊断要求,并能获得较高的SNR。

【Abstract】 Objective:To evaluate the effectiveness of using the automated selection of the optimal cardiac phase function to automatically select the optimal cardiac phase and reconstruct images in the coronary CT angiography(CTA) for patients with arrhythmia. Methods:Thirty-two arrhythmia patients who underwent coronary CTA were retrospectively collected,the variability of their heart rate was more than 20 BPM. CTA image data were obtained with prospective ECG-triggered protocol in single cardiac phase and scanning window was selected based on the recommendation from Auto Gating. CTA images were reconstructed twice using the center phase of scanning window(group A) and the phase recommended by automated selection of the optimal cardiac phase function(group B). The CT value and Noise(SD) of the aortic arch root were measured and SNR was calculated. The main coronary artery ratings were performed independently by two experienced radiologists according to the continuity of the vascular lumen,the sharpness of edges and the motion artifacts with 5-point system. The artery image quality scores and SNR from the two groups were analyzed using the rank sum test and paired sample t-test. Result:The SNR of the aortic arch root of the two groups was significanly different(P< 0.05). The image quality scores of the right coronary artery,the left coronary artery,the left circumflex and the left anterior descending artery territory of two groups were significantly different(all P< 0.05).Conclusion:Automated selection of the optimal cardiac phase in coronary CTA for arrhythmia patients can provide images that meet the requirements of clinical diagnosis at a high SNR.
