【摘要】 目的探讨以护士为主导的个案管理模式对于糖尿病患者的干预效果。方法便利抽样选择2016年4-9月在社区招募的糖尿病患者共120例,按照随机数字表法分为观察组60例和对照组60例。对照组患者行常规健康教育,观察组患者采用以护士为主导的个案管理模式进行干预,时间均为12周。比较两组患者的血糖控制、自护行为以及授权能力。结果观察组患者的餐前餐后血糖、氨基糖化血红蛋白(HbA1c)水平低于对照组患者,自护行为和授权能力的得分高于对照组,差异均有统计学意义(均P<0.05)。结论 12周的以护士为主导的个案管理模式可以降低2型糖尿病患者血糖水平,增强其自护行为,同时提高相应的授权能力。更多还原

【Abstract】 Objective To explore the intervention effect of nurse-led case management model on diabetic patients.Methods By convenience sampling,120 cases with type 2 diabetes mellitus were recruited in a community from April to September 2016,and randomly divided into observation group(n=60)and control group(n=60).The control group received routine health education for 12 weeks,while the observation group received 12-week nurse-led case management.The glycemic control,self-care behavior and empowerment were compared between two groups.Results The levels of preprandial blood glucose(PPBG)and postprandial blood sugar(PPBS)and HbA1 cin observation group were significantly lower than those in the control group,while the scores of self-care behavior and empowerment in observation group were significantly higher than those of control group(all P<0.05).Conclusions The 12 weeks of nurse-led case management programe can lower the blood glucose level of patients with with type 2 diabetes mellitus,improve their self-care behaviors and enhace the ability of empowerment.更多还原
