【摘要】 目的了解2型糖尿病(type 2diabetes mellitus,T2DM)患者低血糖恐惧感(fear of hypoglycemia,FoH)的现状,并探讨其影响因素。方法 2016年5-11月,采用便利抽样法选择在长春市3所三级甲等医院内分泌科住院的260例T2DM患者为研究对象,采用一般资料调查表、中文版低血糖忧虑量表(the Chinese version of the worry scale,C-WS)对其进行调查。结果 260例T2DM患者C-WS得分的中位数是11分,四分位间距是7~20分;单因素分析结果显示,不同年龄、性别、居住方式、病程、治疗方式、血糖波动情况、糖尿病控制状况、社会支持、低血糖发生频率、低血糖严重程度、低血糖意识的T2DM患者的低血糖恐惧感得分差异均有统计学意义(均P<0.05);多元逐步回归分析结果显示,病程长、独居、血糖波动较大、社会支持较少、过去6个月内发生过3~6次低血糖、过去6个月内低血糖发生次数>1次/月、中度低血糖、重度低血糖是T2DM患者FoH得分高的影响因素。结论 T2DM患者对低血糖存在担忧、恐惧心理,且这种心理状态可受到病程、居住方式、血糖波动情况、社会支持状况、低血糖发生频率、低血糖严重程度的影响;提示医护人员应重视对T2DM患者低血糖方面的健康教育,同时应根据患者的不同特点,制订个体化的干预方案,以缓解患者的FoH,改善其健康状况。更多还原

【Abstract】 Objective To investigate the status quo of fear of hypoglycemia(FoH)in patients with type 2 diabetes mellitus(T2DM),and to explore its influencing factors.Methods By convenience sampling method,260 cases with T2DM from the department of endocrinology of three Class Ⅲ GradeⅠhospital in Changchun during the period of May to November in 2016 were selected and investigated by using general scale and the Chinese version of the worry scale(C-WS).Results The median score of C-WS was 11,and the interquartile range was 7-20.The results of univariate analysis showed that patients with different age,gender,ways of living,diabetes cours,treatment modalities,blood glucose fluctuation,diabetes control,social support,frequency and severity of hypoglycemia,and awareness of hypoglycemia had statistically significant difference in terms of scores of FoH(all P<0.05).The results of multiple stepwise regression analysis demonstrated that course of diabetes,ways of living,higher blood glucose fluctuation,less social support,3-6 times of hypoglycemia in the past 6 months,more than one time of hypoglycemia per month in the past 6 months,moderate and severe hypoglycemia were influencing factors of scores of FoH in patients with T2DM.Conclusions Patients with T2DM are worried about hypoglycemia,and this mental state is affected by diabetes duration,ways of living,blood glucose fluctuation status,social support status,frequency of hypoglycemia and degree of hypoglycemia.Health care workers should pay attention to health education on hypoglycemia,at the same time,they should develop individualized intervention programs based on patients’ different characteristics,so as to alleviate their FoH and improve their physical and mental health status.更多还原
