磁敏感加权成像对新生儿缺氧缺血性脑病磁共振常规序列分度的评价

【摘要】 目的分析磁敏感加权成像(SWI)与新生儿缺氧缺血性脑病MRI常规序列分度的相关性。方法方便收集2015年1月—2017年12月该院经临床确诊的新生儿缺氧缺血性脑病患儿63例,所有患儿均行MRI常规序列及SWI序列检查。比较常规MRI序列与SWI序列对颅内出血、脑内异常静脉扩张的检出情况。比较不同临床分度新生儿缺氧缺血性脑病患儿的SWI表现。结果 MRI平扫在47例患儿中共检出出血灶T1WI序列79处、T2WI序列50处、FLAIR序列93处,SWI检出出血灶147个;T1WI、T2WI、FLAIR序列均未明确显示扩张静脉影,SWI在38例患儿中检出脑内异常扩张静脉81处;SWI序列对轻、中、重度新生儿缺氧缺血性脑病颅内出血、异常静脉扩张及颅内出血合并静脉扩张检出率差异有统计学意义(P<0.05),其中重度组检出率最高,轻度组检出率最低。结论 SWI显示新生儿缺氧缺血性脑病患儿颅内出血及脑内异常静脉扩张的敏感性高于常规MRI序列,可更准确地评估新生儿缺氧缺血性脑病患儿的病情轻重程度。

【Abstract】 Objective To analyze the correlation between magnetic susceptibility-weighted imaging(SWI) and MRI routine sequence grading of neonatal hypoxic-ischemic encephalopathy. Methods A total of 63 children with neonatal hypoxic-ischemic encephalopathy diagnosed clinically in the hospital from January 2015 to December 2017 were convenient collected.All patients underwent routine MRI and SWI sequence examinations. The detection of intracranial hemorrhage and abnormal venous dilation in the brain was compared between the conventional MRI sequence and the SWI sequence. The SWI performance of children with neonatal hypoxic-ischemic encephalopathy in different clinical grades was compared. Results 47 T1 WI sequences, 50 T2 WI sequences, and 93 FLAIR sequences were detected in 47 cases of MRI scans. 147 hemorrhage lesions were detected on SWI. T1 WI, T2 WI, and FLAIR sequences did not clearly show dilated veins, SWI detected 81 abnormally dilated veins in 38 patients; SWI sequence showed differences in detection rates of intracranial hemorrhage, abnormal venous dilatation and intracranial hemorrhage with venous dilation in mild, moderate and severe neonatal hypoxic-ischemic encephalopathy. There was statistical significance(P<0.05), with the highest detection rate in the severe group and the lowest detection rate in the mild group. Conclusion The sensitivity of intracranial hemorrhage and abnormal venous distension in the brain of children with neonatal hypoxic-ischemic encephalopathy is higher than that of conventional MRI sequences, which can more accurately assess the severity of neonatal hypoxic-ischemic encephalopathy in children.
