经桡动脉介入术后螺旋式止血器非闭塞性压迫法研究

【摘要】 目的探讨经桡动脉冠脉介入术后应用改良式非闭塞性压迫法的方法及效果。方法将经桡动脉行冠状动脉支架置入术后应用螺旋式桡动脉压迫器患者200例,随机分为对照组和干预组各100例。对照组采用常规护理方法;干预组应用非闭塞性桡动脉压迫法,即以压迫尺动脉后术侧拇指的SpO2≥0.95作为标准调节桡动脉止血器的压力强度,从而保证在加压过程中桡动脉一直处于开放状态。比较两组术侧肢体肿胀、疼痛程度及穿刺局部出血情况。结果两组均未出现严重出血情况,术后4h与术前术侧上肢各手指第2节周径差值差异有统计学意义(P<0.01);干预组疼痛及术侧肢端肿胀程度显著低于对照组(均P<0.01)。结论改良式非闭塞性压迫止血法既能及时减压而又不增加出血的风险,能有效预防PCI术后并发症,提高患者的舒适感。

【Abstract】 Objective To observe the effect of modified non-occlusive compression method on hemostasis at the puncture site after percutaneous coronary intervention.Methods A total of 200 patients using the spiral radial artery compression hemostat after coronary stenting through the radial artery were randomly divided into a control group and an intervention group of 100 cases each.The control group received routine occlusive compression at the puncture site.The intervention group received non-occlusive compression,that is,the pressure intensity of the radial artery compression hemostat was adjusted by making SpO2 of the thumb of the operated limb 0.95 or higher after compressing the ulnar artery,with a purpose to ensure that the radial artery was kept open during the compression process.The differences in limb swelling,pain,and bleeding at the puncture site were compared between the two groups.Results There was no severe bleeding in the two groups;the degrees of pain and swelling at the operated limb in the intervention group were significantly milder than those in the control group(P<0.01 for all).Conclusion Modified non-occlusive compression hemostasis method can not only reduce pressure at the compression site without increasing the risk of bleeding,but also prevent the occurrence of complications and improve patient comfort levels.
