【摘要】 目的探讨关节镜辅助下内引流术治疗腘窝囊肿的疗效。方法 2015年2月-2018年5月,采用关节镜内引流技术对20例腘窝囊肿患者进行治疗,其中男性6例,女性14例,年龄55~78岁,平均61.7岁。腘窝囊肿均为单侧,其中右膝12例,左膝8例。在处理关节内疾患的同时,辅助后内侧关节镜入路,扩大囊肿开口至1.0 cm,使囊肿与关节相交通,根据Rauschning-Lindgren分级方法评定手术效果。结果 20例腘窝囊肿患者在术中均发现伴有关节内疾患,其内侧半月板撕裂15例,外侧半月板撕裂2例,骨性关节炎19例,痛风性关节炎1例。术后未出现血管、神经或手术切口感染等并发症。术后1~3天出院。随访时间6~27个月,平均13个月。1例囊肿复发。术后膝关节不适症状均较术前明显缓解。Rauschning-Lindgren评分明显低于术前(P<0.01)。结论关节镜辅助下内引流术治疗窝囊肿具有操作简便、安全、创伤小、恢复快、复发率低、可同时处理关节内疾病的优点。更多还原

【Abstract】 Objective Treatment of popliteal cyst with arthroscopic internal drainage. Methods From February2015 to May 2018,20 patients with popliteal cyst were treated with arthroscopic internal drainage technique,including 6 males and 14 females,aged from 55 to 78 years( mean 61. 7 years). All the cysts were unilateral,including 12 cases of right knee and 8 cases of left knee. The internal articular diseases were treated with the posterior medial arthroscopic approach,the opening of the cyst was enlarged to 1. 0 cm,and the communication between the cyst and the joint was made. The operative effect was evaluated according to the Rauschning-Lindgren classification method. Results All the20 cases of popliteal cyst were found to have intraarticular diseases during the operation,including 15 cases of medial meniscus tear,2 cases of lateral meniscus tear,19 cases of osteoarthritis and 1 case of gouty arthritis. There were no complications such as infection of blood vessels,nerves or surgical incision after operation. The patients were discharged1 ~ 3 days after operation. The follow-up time was 6 ~ 27 months,with an average of 13 months. Postoperative symptoms of knee joint discomfort were significantly alleviated after operation. Conclusions Arthroscopic internal drainage has the advantages of simple operation,safety,small trauma,quick recovery and low recurrence rate. It can also be used to treat intra-articular diseases.更多还原
